Phaco-trabeculectomy in advanced, controlled, open-angle glaucoma patients with cataract results in better postoperative visual field MD with no major adverse events.
BackgroundBeta-thalassemia is a severe genetic blood disorder caused by a mutation in the gene encoding for the beta chains of hemoglobin. Individuals with beta-thalassemia major require regular lifelong Red Blood Cell transfusions to survive. Ocular involvement is quite common and may have serious implications.MethodsExtensive review of observational studies on beta-thalassemia, to determine the prevalence and spectrum of ocular abnormalities, by clinical examination and multimodal imaging, and to investigate risk factors for their development.ResultsFrequency of ocular involvement differs among various studies (41.3–85 %, three studies). Ocular findings in beta-thalassemia may correlate to the disease itself, iron overload or the chelating agents used. Beta-thalassemia ocular manifestations include ocular surface disease, as demonstrated by tear function parameters (two studies). Lens opacities are present in 9.3–44 % (five studies). Lenticular opacities and RPE degeneration correlated positively with use of desferrioxamine and deferriprone respectively (two studies). Ocular fundus abnormalities characteristic of pseudoxanthoma elasticum (PXE), including peau d’orange, angioid streaks, pattern dystrophy-like changes, and optic disc drusen are a consistent finding in seven studies. Patients with PXE-like fundus changes were older than patients without these fundus changes (two studies). Age (two studies) and splenectomy (one study) had the strongest association with presence of PXE-like fundus changes. Increased retinal vascular tortuosity independently of the PXE-like fundus changes was found in 11–17.9 % (three studies), which was associated with aspartate amino transferase, hemoglobin and ferritin levels (two studies). Fundus autofluorescence and electrophysiological testing (ERG and EOG) may indicate initial stages or more widespread injury than is suggested by fundus examination (two studies).ConclusionsBeta-thalassemia may present with various signs, both structural and functional. Pseudoxanthoma elasticum like fundus changes are a frequent finding in patients with b-thalassemia. These changes increase with duration or severity of the disease. Retinal vascular tortuosity may be an additional disease manifestation related to the severity and duration of anemia and independent of the PXE-like syndrome. Patients with long-standing disease need regular ophthalmic checkups because they are at risk of developing PXE-like fundus changes and potentially of subsequent choroidal neovascularization.
Purpose glaucoma patients with borderline controlled intraocular pressure (IOP) may benefit from cataract surgery alone. However it is possible that these patients may need glaucoma surgery at some point in the future. Furthermore the risk of postoperative IOP spikes and the inconvenience of two operations (sequential) should be weighed against the complexity of the combined operation. The purpose of the study is to compare the results of phacotrabeculectomy with trabeculectomy in clear corneal incision pseudophacic eyes. Methods retrospective study of 31 patients with visually significant cataract and open angle glaucoma with IOP 22‐25 mmHg on topical medications. The patients were assigned to either surgical treatment according to their glaucoma severity. 20 patients underwent combined phacotrabeculectomy (two‐sites) and 11 patients underwent sequential (1st phacoemulsification(temporal clear corneal incision with intraocular lens implantation), 2nd trabeculectomy). Antimetabolites and adjustable sutures were used in all glaucoma operations. IOP measurements were recorded. Follow up time was 6‐18 months (median 12 months). Results the effect of type of surgical treatment on postoperative IOPs was not statistically significant. Combined phacotrabeculectomy resulted in higher IOPs by 0.97(95% Confidence Interval [‐1.6 3.59]) mmHg (p=0.47) versus sequential surgery. Conclusion in patients with clinically significant cataract and glaucoma with borderline control of IOP the choice of combined (phacotrabeculectomy) versus sequential (1st phaco, 2nd trabeculectomy) surgery can be done according to the severity of glaucoma and based on surgeon’s preference and experience.
Σκοπός: Η σύγκριση των αποτελεσμάτων και της μεσοπρόθεσμης μετεγχειρητικής πορείας της συνδυασμένης φακοθρυψίας-τραμπεκουλεκτομής έναντι της απλής φακοθρυψίας σε ασθενείς με κλινικά σημαντικό καταρράκτη και προχωρημένο ή τελικού σταδίου γλαύκωμα ανοικτής γωνίας ρυθμιζόμενο υπό τοπική αντιγλαυκωματική αγωγή και η ανάδειξη προγνωστικών παραγόντων της μετεγχειρητικής έκβασης.Ασθενείς και μέθοδος: προοπτική, παρεμβατική, παράλληλη τυχαιοποιημένη κλινική μελέτη. 60 ασθενείς με κλινικά σημαντικό καταρράκτη (Μέση Απόκλιση των οπτικών πεδίων χειρότερη από -15dB), ελεγχόμενη Ενδοφθάλμια Πίεση(σταθερά χαμηλότερη από 22mmHg) υπό τοπική αντιγλαυκωματική αγωγή και χωρίς προηγηθείσα οφθαλμική επέμβαση υποβλήθηκαν με 1: 1 τυχαιοποίηση σε συνδυασμένη επέμβαση φακοθρυψίας-τραμπεκουλεκτομής είτε σε απλή φακοθρυψία. Η ανάλυση των μετεγχειρητικών αποτελεσμάτων έγινε ανάλογα με την θεραπευτική πρόθεση(Intention-to-Treat Analysis) και η διερευνητική ανάλυση για την ανίχνευση προγνωστικών παραγόντων για την μετεγχειρητική Ενδοφθάλμια Πίεση, τη βελτίωση της Οπτικής Οξύτητας, τη μεταβολή των Οπτικών Πεδίων και τη μείωση του αριθμού των αντιγλαυκωματικών κολλυρίων έγινε με γραμμική παλινδρόμηση διαχρονικών δεδομένων έως και 2 έτη μετεγχειρητικά. Η μελέτη ήταν τυφλή ως προς την αρχική κατανομή των ασθενών σε έκαστη θεραπευτική ομάδα καθώς για τους συλλέκτες των δεδομένων. Αποτελέσματα: 31 και 29 ασθενείς τυχαιοποιήθηκαν στην ομάδα απλής φακοθρυψίας και στην ομάδα συνδυασμένης φακοθρυψίας-τραμπεκουλεκτομής αντίστοιχα. Οι ασθενείς της συνδυασμένης επέμβασης είχαν κατά 1.7mmHg [95%Διάστημα Εμπιστοσύνης: -3.1 έως -.23] μεγαλύτερη μείωση της ΕΟΠ, κατά 1.4dB [95%ΔΕ:-.17 έως 2.96] μεγαλύτερη βελτίωση της Μέσης Απόκλισης (MD) των οπτικών πεδίων, κατά 0.6[95%ΔΕ: -1.2 έως -.05] μεγαλύτερη μείωση του αριθμού των αντιγλαυκωματικών κολλυρίων μετεγχειρητικά ενώ η βελτίωση της οπτικής οξύτητας ήταν παρόμοια μεταξύ των 2 θεραπευτικών ομάδων. Σημαντικότεροι προγνωστικοί παράγοντες για τους κύριους δείκτες μετεγχειρητικής έκβασης αναδείχτηκαν: (α) για τη μετεγχειρητική ΕΟΠ: η συνδυασμένη επέμβαση φακοθρυψίας τραμπεκουλεκτομής και η απόκλιση από τη μέση ΕΟΠ του δείγματος προεγχειρητικά, (β) για τη Μέση Απόκλιση των Οπτικών Πεδίων μετεγχειρητικά: ο βαθμός σκληρότητας του πυρήνα, η παρουσία παθολογικού φωτοκινητικού αντανακλαστικού και η απόκλιση από τη μέση Μέση Απόκλιση(MD) και το μέσο λόγο κοίλανσης-προς-διάμετρο οπτικού δίσκου των ασθενών της μελέτης προεγχειρητικά, (γ) για την οπτική οξύτητα μετεγχειρητικά: προεγχειρητική οπτική οξύτητα και παρουσία παθολογικού φωτοκινητικού αντανακλαστικού, (δ) για τον αριθμό αντιγλαυκωματικών κολλυρίων: η συνδυασμένη φακοθρυψία-τραμπεκουλεκτομή, ο προεγχειρητικός αριθμός αντιγλαυκωματικών κολλυρίων και η απόκλιση από τη μέση Μέση Απόκλιση (MD) προεγχειρητικά. Η ομάδα φακοθρυψίας παρουσίασε συχνότερες και μεγαλύτερες σε μέγεθος οξείες υπερτονίες (>25mmHg)Odds Ratio(OR)0.34[95%ΔΕ: 0.11 1.02]. Απώλεια της αντίληψης φωτός δεν παρατηρήθηκε σε κανέναν ασθενή. Συμπεράσματα: Σε ασθενείς με κλινικά σημαντικό καταρράκτη και προχωρημένο γλαύκωμα ανοικτής γωνίας ρυθμιζόμενο με τοπική φαρμακευτική αγωγή η συνδυασμένη επέμβαση φακοθρυψίας-τραμπεκουλεκτομής παρέχει καλύτερη Μέση Απόκλιση (MD) των οπτικών πεδίων μετεγχειρητικά χωρίς σημαντικά ανεπιθύμητα αποτελέσματα.
