Case presentationA 65-year-old man was admitted to the intensive care unit (ICU) of Tenon hospital, a university teaching hospital in Paris, France, for severe confirmed COVID-19 pneumonia. He had a history of diabetes. In the preceding week, the patient developed fever (39 C), cough, and shortness of breath. A chest computed tomography (CT) scan showed bilateral ground glass opacities with alveolar consolidation (Figure 1A) predominant in the lung periphery as usually reported in COVID-19 pneumonia (Chung et al. 2020;Jajodia et al. 2020). Advanced life support was administered including lung-protective ventilation, prone-positioning, and vasoconstrictors. Empirical antimicrobial therapy was initiated, in association with hydrocortisone. The infusion of Tocilizumab was administered (Zhang et al. 2020). Introduction
Le KL-6 est une glycoprotéine des pneumocytes de type II, qui a montré son intérêt dans le diagnostic et l’évaluation de la sévérité de nombreuses pneumopathies interstitielles diffuses. Dans la pneumopathie à SARS-CoV-2, plusieurs biomarqueurs sanguins (CRP, D-Dimères, LDH) ont été évalués pour aider à la stratification des patients. L’objectif de notre étude est de déterminer la valeur pronostique du taux de KL-6 initial dans l’infection à SARS-CoV-2.
Méthodes
Ont été inclus tous les patients hospitalisés pour suspicion de pneumopathie à SARS-CoV-2 dans les services de pneumologie et de réanimation d’un hôpital universitaire francilien (du 5/03 au 5/05/20). Les patients ont été classés selon l’échelle de sévérité de l’OMS

. La concentration sanguine initiale du KL-6 a été mesurée dans les 72 h suivant la suspicion diagnostique. Les analyses de survie ont été réalisées selon un modèle de régression de Cox et modélisées par une courbe de Kaplan-Meier.
Résultats
81 sur 221 malades hospitalisés (37%) disposant d’un dosage de KL-6 ont été analysés, dont 66 (47 hommes, âge: 66 ans ± 14 ans) qui présentaient une Covid-19 et 15 témoins (PCR négative et diagnostic alternatif). La concentration (moyenne ± DS) de KL-6 initial était de 409 ± 312 U/mL chez les Covid-19 vs. 362 ± 161 U/mL chez les témoins (NS). Le KL-6 était supérieur à la normale (≤ 500 U/ml) chez 36% des Covid-19 et 27% des témoins (NS). Une valeur de KL-6 initial élevée était significativement associée à la mortalité intra-hospitalière (HR: 8,66; IC 95%:1,1–69,2), à l’admission en réanimation (
p
= 0,049) et à l’extension radiologique des lésions au scanner (
p
= 0,0040). Le KL-6 initial était plus élevé dans les groupes de sévérité OMS ≥ 6 par rapport au groupe OMS ≤ 5 (
p
= 0,042). Chez les patients décédés (13%), le KL-6 augmentait alors que qu’il restait stable ou diminuait chez les survivants. À 3 mois (

= 48), la DLCO était négativement corrélée à la valeur du KL-6 initial (

= −0,47,

= 0,001), ce qui n’était pas le cas de la CVF, de la VEMS et du score mMRC.
Conclusion
La valeur du KL-6 initial est associée à la mortalité hospitalière, à une évolution défavorable et à une altération persistante de la DLCO à 3 mois. La valeur du KL-6 initial semble avoir un intérêt pronostique dans la pneumopathie à SARS-CoV-2.
