Ateneo, 1940.O problema das inapetências é dos mais encontrados quer na clínica como manifestação nervosa aquilo que não é sinão um sintoma de alguma moléstia em evolução.Para demonstrar a importância deste problema é citada uma estatística de Bartlet na qual se verifica que de 1471 pacientes examinados, em 349 a queixa principal referia-se à anorexia, o que representa uma proporção extraordinariamente grande. Tanto na clínica hospitalar como na particular, as anorexias concorrem com cerca de 20% dos casos na maioria das estatísticas. A comparação entre os dados estatísticos da clínica hospitalar e particular demonstra um fato muito interessante. Na primeira notamos que entre as 29 causas capazes de determinar o sintoma anorexia, a neuropatia ocupa o 12.° lugar com 10 casos sobre 360, enquanto que, na segunda, seu papel é muito mais importante, porquanto ocupa o primeiro lugar entre as causas determinantes das inapetências, com 10 casos sobre 57.Escardó estuda o ato. de comer, ato de inicio correspondente a uma necessidade orgânica geral e que, posteriormente, se torna um ato volitivo influído por fatores emocionais, mecânicos, físicos, sensoriais, estéticos, emotivos, educacionais, sociais e patológicos. Todos estes fatores são considerados como capazes de alterar a função normal de comer, sendo preciso em cada caso notar o ponto da cadeia que está alterado. A fome é um reflexo trófico desencadeado por uma necessidade celular fisiológica, indiferenciada que irá funcionar como estimulo para desencadeamento das atividades capazes de satisfazer as necessidades plasmáticas. Comprende-se que de inicio, em tese, todo o elemento intelectual está excluido deste reflexo, porem, como esta necessidade nem sempre pode ser satisfeita de uma maneira simples, o estimulo tende a se difundir, aparecendo então na conciência onde será reconhecido como sensação. O apetite será, então, um fenômeno conciente ligado estreitamente a uma experiência trófica, isto é, a noção que o indivíduo adquire empiricamente que lhe permite reconhecer que uma determinada substancia caracterisada por uma forma, côr, aspecto e consistência particulares tem a capacidade de acalmar a fome cada vez que seja ingerida. É por isto, por exemplo, que um prato exótico que não conhecemos, não nos dá a impressão que poderá satisfazer a nossa fome.A fome, como manifestação da fase catabólica do metabolismo, é um estímulo para experiências tróficas destinadas a satisfazer as necessidades fisiológicas da célula neste período. O ato de comer, entretanto, habitualmente é um reflexo condicionado; a condicionalidade deste ato faz aparecer não somente a sensação de apetite como também os fenômenos motores e secretorios que acompanham o ato de comer. As imposições da vida social civilisada, a subordinação aos horários e ao ritmo de vida comum, não permitem que se espere a sensação de fome para se realizarem as atividades destinadas a suprimir esta situação.
