Introduction. The use of mathematical modeling methods in clinical practice will make it possible to identify the pathogenetic forms of dorsopathies and thus reasonably use the concept of targeted treatment in the management of patients of this category.Aim. To evaluate the possibilities of finite element segmental analysis of the cervical spine for personalized treatment and prediction of the course of dorsopathies.Material and methods. Based on the combined data of computed and magnetic resonance imaging of the patient (female, born in 1951), a model of the C5 – C7 segment was generated, including: vertebrae C5, C6, C7, IVD, anterior and posterior longitudinal ligaments, two pairs of facet joints, spinal cord, nuchal ligament. Computer modeling and finite element method were used to analyze the stress-strain state of the cervical spine of a patient with degenerative-dystrophic changes in the C2 – C7 segments. In the Abaqus/CAE 6.14 software, finite element analysis of the C5 – C7 stress-strain state was carried out in the state of flexion, rotation and compression. The data obtained during compression were compared with previous experiments in silico and in vitro for the norm.Results. For each state, stress and displacement diagrams, load-displacement curves, stress profiles in the MPD were obtained. The axial mobility of the segment under compressive load is two times lower compared to the norm under the same boundary conditions and material models. The degree of involvement of the spinal cord in conflicts with the surrounding anatomical structures was studied. When the model was rotated to the right, conflicts were observed between the spinal cord roots and the bone structures of the vertebrae in the foraminal zones, as well as at the level of the C5 – C6 and C6 – C7 discs with the left posterolateral surfaces of the fibrous rings. When the model was turned to the left, conflicts of the spinal cord were observed in all foraminal zones, as well as at the level of the C6 – C7 disc with the left posterolateral surface of the fibrous rings. Based on the data on stresses in the studied segment, further development of dorsopathies and degenerative changes in the cervical spine was predicted.Conclusions. The use of finite element segmental analysis of the cervical spine creates objective prerequisites for the formation of a combined personalized treatment and prediction of the course of dorsopathies.
Rationale: 3D modeling of various anatomical structures has recently become a separate area of topographical, anatomical, and biomechanical studies. Current in vivo visualization methods and quantitative analysis in silico allow to perform the precise modeling of these processes aimed at investigation into the pathophysiology of cardiovascular disorders, risk prediction, planning of surgical interventions and virtual refinement of their separate stages.Aim: To develop tools for elaboration, analysis and validation of personalized models of various structures of the heart and aortal arch taking into account their morphological characteristics.Materials and methods: We used the results of 14 computed tomography studies from randomized patients without any disease or anomaly of the heart, aortic valve and aortal bulb. The analysis and subsequent transformation of the images were done with Vidar DICOM Viewer, SolidWorks 2016, VMTKLab software. For the FSI modeling of the aortic arch based on the results of functional multiaxial computed (MAC) coronarography (a female patient of 55 years) we developed a personalized model of the ascending aorta and aortic arch at the beginning of the systole. Using HyperMesh software (Altair Engineering Inc., USA) we have built a network of finite element of the luminal area, adventitia, and aortic media. To model mechanical properties of the aortic structures we used an anisotropic hyperelastic material model by Holzapfel – Gasser – Ogden. Material modeling, choice of the limiting antecedents, and analysis of fluid-structure interaction were performed with Abaqus CAE 6.14 software (Simulia, Johnston, USA). Adaptive image meshing by Young was used to elaborate the finite element template of the left ventricle. The algorithm was realized within the IDE PyCharm software media in Python 3.7. The algorithm was realized based on the open-source libraries OpenCV, NumPy, Matplotlib, and SciPy.Results: The first stage of the development of the aortic valve model included the design of its virtual 3D template. Thereafter, a cohesive geometric model was elaborated. Subsequent stage of the work included the transformation of the aortic valve geometric model into the parametric one. This was done through the use of the “Equations” tool within the SolidWorks. No problems with geometry of the model during its deformation were identified. Aortic segment modeling was based on the data obtained by functional MAC coronarography. Based on this and on Inobitec Dicom Viewer software, we generated a multiplane reconstruction of the zone of interest including anatomical structure of the heart and aortic valve. With the resulting set of contours, we created a 3D model, which then was converted into a polygonal stereolithographic model. We developed an algorithm for adaptive meshing to elaborate a polygonal template capable of deformation that can be used for registration both with the net methods (B-Spline) and based on the image characteristics (homologous pixels). Conclusion: The resulting parametric 3D model of the aortic valve anatomical structures is capable of adequate transformation of its geometry under external factors. It can be used in simulators of endovascular cardiosurgical procedures.
РЕЗЮМЕ Актуальность. Злокачественные опухоли органов головы и шеи составляют 20-25% случаев всей онкологической патологии в Российской Федерации. При этом частота обнаружения злокачественных опухолей полости рта составила 27,2 на 100 тыс. населения в 2017 г. Пик заболеваемости у мужчин и женщин наблюдается в возрасте 59,7-63,9 года, что включает трудоспособную группу населения. За последние несколько лет отмечается неуклонный рост числа пациентов с данной патологией. Несмотря на значительный прогресс в лечении онкологических пациентов в целом, 5-летняя безрецидивная выживаемость больных этой группы существенно не изменилась за последние 20 лет и составляет около 45-50%. Цель исследования. Разработка алгоритма и программного комплекса для автоматического выявления структур, подозрительных на злокачественные новообразования в области нижней челюсти, методом анализа и сегментации изображений, полученных при компьютерной томографии с помощью технологий глубокого обучения. Материал и методы. Для классификации фрагментов изображения применяется классификационная сверточная нейронная сеть с архитектурой U-net. В целях обучения и контроля сети использовали КТ 22 пациентов с опухолями нижней половины лицевой области, состоящие из 383 размеченных срезов размером 512×512 пикселей. Разметку производили квалифицированные челюстно-лицевые хирурги и команда радиологов. Сформированную в результате классификации карту вероятностей обрабатывали с целью получения бинарной маски патологического участка. Оценку сети производили на контрольной выборке с применением метрики IoU. Результирующие маски имели коэффициенты IoU в диапазоне от 0.6 до 0.76. Результаты. На контрольной выборке (50 изображений) алгоритм показал попадание в зону новообразования в 98% случаев со средней точностью контурирования 0,68 по метрике IoU. Итоговый результат работы тестирования алгоритма представлен в нижеприведенных иллюстрациях. Для сравнения также представлены результаты ручной разметки КТ-изображений, которые использовали при проведении «контроля», а соответственно, не предъявляли ранее ИНС. Заключение. В результате проведенного исследования разработан программный комплекс на основе обученной искусственной нейронной сети, способный с высокой долей точности выполнять автоматический анализ и сегментацию полученных при компьютерной томографии изображений нижней челюсти, выявляя злокачественные образования данной области и границы их распространения. В клинической практике разработанное программное обеспечение можно применять для автоматического поиска подозрительных на злокачественную опухоль объектов с последующим углубленным изучением этой области специалистом, т.е. использовать как систему поддержки принятия решения хирургом и/или врачом лучевой диагностики с целью экономии рабочего времени на оценку данных компьютерной томографии.
Introduction. In the structure of the global burden of diseases, back pain ranks first in the world in the list of causes of disability. Neck pain is one of the most common diseases of the musculoskeletal system. An integral part of the strategy for the treatment of acute neck pain is the use of original nonsteroidal anti-inflammatory drugs (NSAIDs) with high proven effectiveness.Objective. To conduct a comparative analysis of the therapeutic efficacy of the original drug meloxicam (Movalis) and its generic (Amelotex) in the treatment of patients with acute nonspecific musculoskeletal pain of cervical localization.Materials and methods. The article provides an assessment of the therapeutic effectiveness of the original drug meloxicam (Movalis) and its generic (Amelotex) in the treatment of patients with acute neck pain. Comparative analysis of the results of treatment of 108 patients hospitalized in the Branch of the MC JSC “Admiralteyskie Verfi” aged 18 to 60 years (58 men and 50 women) with a diagnosis of acute dorsalgia of cervical localization (M54.2 according to ICD-10). The average age of patients is 42.5 ± 11.1 years.Results. As a result of the clinical and neurological study and statistical processing of the data obtained, it was found that the use of Movalis in the treatment of patients with acute neck pain was more effective compared to the use of generic (Amelotex). In the Movalis group, the level of pain intensity and the index of disability after treatment was significantly lower than in the Amelotex group at an equivalent dose of 15 mg per day (p < 0.01). The analgesic effect in the group of patients receiving Movalis was longer. There was a significant decrease in sleep disorders associated with pain syndrome in the group (Movalis) compared with patients of the second group (Amelotex). Conclusions. The absence of therapeutic bioequivalence between Movalis and Amelotex revealed in our study determines the relevance of the choice of drugs for the complex therapy of patients of this profile. A multimodal approach to the strategy of treating neck pain, individually patient-oriented, including the use of effective and safe medicines, physical exercises, manual therapy, is optimal.>< 0.01). Study objective — To evaluate how morphological features of intervertebral disc would affect the outcomes of finite element modeling of axial load in the cervical spine, C3-C5, in order to predict the risk of occurrence and course of dorsopathies. Material and Methods — Three-dimensional models of the cervical spine vertebrae were generated from the computed tomography data of a volunteer (24 years old male without detected pathology of his neck). Intervertebral disc models were developed in two configurations. For each model, we performed a finite element investigation of the stress-strain state with the same loading conditions. The load-displacement curves were compared with the experimental data generated from the results of previously conducted in vitro experiments. Results — The maximum and mean displacement values for the isotropic model were 1.15 mm and 0.73 ± 0.45 mm, respectively. For anisotropic model, maximum and mean displacement values were 0.86 mm and 0.47 ± 0.24 mm, correspondingly. Predicted displacement values for both models matched the experimental data fairly well. Stress profiles of intervertebral discs and stress diagrams of facet joints were calculated. Conclusion — The proposed geometric and constitutive configurations of the intervertebral disc take into account specific morphological features at low computational costs, thereby facilitating the modeling of degenerative disc changes.
РЕЗЮМЕ Обоснование. Одной из значимых причин преждевременной смертности и инвалидизации населения в настоящее время является патология клапанов сердца. Наибольшее распространение среди этой группы патологий имеет аортальный стеноз. Самым востребованным методом его коррекции на сегодняшний день является транскатетерная имплантация протеза аортального клапана. Для обучения ординаторов и мануального тренинга чаще используют анатомический материал и операции на животных, однако подобного рода практика имеет высокую стоимость и не всегда доступна широкому кругу обуча ющихся. В качестве альтернативы на рынке появляются виртуальные хирургические симуляторы, включающие рабочую станцию с манипуляторами и программное обеспечение для компьютерной визуализации, однако на сегодняшний день на рынке не представлено готового решения для сегментации структур сосудистого русла, участвующих в транскатетерной имплантации протеза аортального клапана. Цель исследования. Разработка алгоритма для автоматической сегментации структур артериального сосудистого русла, в зоне транскатетерной имплантации протеза АК на основе обработки данных компьютерной томографии. Материал и методы. В ходе работы были использованы 14 КТ-исследований рандомизированных больных, не имеющих аномалий и/или патологии АК и луковицы аорты. Их анализ и дальнейшие преобразования были осуществлены с помощью ПО Vidar DICOM Viewer, SolidWorks 2016, VMTKLab и других существующих видов ПО и библиотек. В связи с тем, что визуализация полулунных заслонок АК при выполнении КТ практически невозможна, было выполнено построение виртуального 3D-шаблона АК с учетом параметров, приведенных в литературных источниках. Результаты. В результате первой части работы был получен готовый виртуальный 3D-шаблон АК, способный изменять свою геометрию при внешних воздействиях. Была доказана работоспособность параметрической модели. В ходе работы с моделью каких-либо проблем с геометрией при деформации выявлено не было. В результате второй части работы была сформирована готовая STL-модель сосудов от устья аорты до уровня перехода наружной подвздошной артерии в бедренную артерию. После этого мы импортировали модель сосудов в SolidWorks и применили параметры механических свойств биологических материалов. На последнем этапе производилась сборка двух моделей в единую систему, в результате чего была сформирована готовая STL-модель зоны интереса с сегментированными структурами. Выводы. 1. При реализации разработанного алгоритма автоматической сегментации структур артериального русла использованы готовые пакеты ПО, что позволило реализовать разработанный алгоритм по достаточно простой и оригинальной методике. 2. Учитывая, что оператор задействован в большинстве этапов использования алгоритма, сегментация в разработанном ПО носит полуавтоматический характер, однако выполняемые оператором действия не требуют глубокого погружения в алгоритмы работы ПО и носят поверхностный характер. 3.
