L’élastofibrome est une tumeur bénigne du tissu mou survient essentiellement chez les personnes âgées de plus de 55 ans avec une prédominance féminine. Il survient électivement à l'angle caudal de la scapula (99%). Il est bilatéral dans 10% à 66% des cas. L'etiopathogenie de l'EF reste encore non élucidée. Il s'agissait d'une étude rétrospective au niveau du laboratoire d'anatomie pathologique d'Ibn sina de Rabat durant une période de cinq ans (2007- 2011), concernant 21 cas d'EF. Toutes ces lésions ont été diagnostiquées après coloration standard à l'HE et à partir des dossiers médicaux des patients, nous avons recueilli leurs données cliniques, radiologiques, leur prise en charge thérapeutique et leur suivi. Quizze des patients étaient de sexe féminin, 6 cas étaient de sexe masculin. Au moment du traitement l’âge moyen était de 57,6 ans. Chez 10 patients, la localisation de la tumeur était du côté droit, 6 cas du côté gauche et bilatérale chez 5 patients. La plupart des patients étaient asymptomatiques. La taille des tumeurs variait entre 5 et 14 cm de grand axe. En IRM, leur aspect était caractéristique et l’étude anatomo-pathologique avait confirmé le diagnostic chez tous les patients. L'elastofibrome est le diagnostic le plus probable quand il s'agit d'une localisation infra-scapulaire d'une masse du tissu mou. L'IRM est l'examen clé du diagnostic. Une éventuelle biopsie permettra d'exclure un processus tumoral malin et rassurer les patients asymptomatiques qu'aucun traitement chirurgical n'est nécessaire.
Solitary fibrous tumor is an uncommon neoplasm affecting adults and typically located in the pleura and can also occur in a large number of other extra thoracic sites. We present the case of a solitary fibrous tumor (SFT) of the retroperitoneum and describe their histopathological and immunohistochemical features. The identification of SFT in the retroperitoneum is of importance because its clinico-pathological behaviour is still unclear. The pathologist plays a fundamental role in establishing both the positive and differential diagnosis.
La colite lymphocytaire (CL), entité rare rentrant dans le cadre anatomoclinique des colites microscopiques (CM), est une maladie inflammatoire de la muqueuse colique. Le pathologiste joue un rôle clé dans le diagnostic de ces CL tout en éliminant un certain nombre de diagnostics différentiels. Toutefois, il convient de faire une confrontation anatomoclinique entre l'histologie qui parle et l'information clinique qui donne la clé pour une interprétation correcte. La majorité des travaux récents permet des progrès dans les connaissances histopathogéniques, étiologiques et théra-peutiques des CL. Cependant, le débat quant à la distinction CL/colite collagène (CC), et dans lequel le pathologiste joue un rôle crucial, est toujours d'actualité. Ainsi, le diagnostic de CM est purement anatomopathologique ; il repose sur l'examen anatomopathologique de biopsies coliques systé-matiques avec une corrélation entre les manifestations cliniques du malade, ayant une diarrhée chronique et une endoscopie normale, et l'aspect histologique des biopsies mettant en évidence des lésions de CL. À travers ce travail, nous allons essayer de souligner l'apport du pathologiste dans le diagnostic positif et différentiel des CL et discuter les interactions entre clinicien et pathologiste. The pathologist plays a key role in the positive diagnosis of LC by eliminating a number of differential diagnoses. However, this diagnosis depends on the interaction between the clinician and the pathologist and comparison between histological and clinical findings, which enable correct interpretation. The majority of recent work enhances our knowledge of the histopathogenic, causal and therapeutic factors. However, the debate about the distinction between LC and collagen colitis is still going on, in which the pathologist plays a key role. The diagnosis of microscopic colitis is purely based on the findings of histological examination of systematic colonic biopsies, with a correlation between the clinical manifestations of the patient with chronic diarrhoea and normal endoscopy and the morphology of biopsies of LC lesions. The purpose of our study is to clarify the role and contribution of the pathologist in positive and differential diagnosis of LC and to discuss the interaction between clinicians and pathologists.
