A facomatose pigmentoqueratósica é uma rara variante da síndrome do nevo epidérmico. Esta doença caracteriza-se pela associação de nevo epidérmico com diferenciação sebácea, nevos spilus e anomalias extracutâneas que envolvem sistemas nervoso central, esquelético e renal. Neste relato, discutimos o caso de uma paciente que apresentava desde o nascimento nevo sebáceo extenso na região cefálica, seguindo as linhas de Blaschko, e múltiplos nevos melanocíticos no hemicorpo direito, bem como alterações esqueléticas decorrentes de raquitismo hipofosfatémico. Ressaltamos a importância de se reconhecer a síndrome do nevo epidérmico e suas diferentes formas de apresentação, bem como manifestações cutâneas associadas a doenças osteometabólicas.
RESUMO -Lobomicose ou doença de Jorge Lobo é uma infecção crónica e granulomatosa causada pela implantação traumática do fungo Lacazia loboi. Esta doença acomete pele e tecido celular subcutâneo. Trata-se de uma dermatose infecciosa típica de regiões tropicais e subtropicais que acomete frequentemente homens adultos ativos nas áreas florestais, além de ter sido diagnosticada em golfinhos. No geral afecta áreas expostas da pele, tais como o pavilhão auricular, os membros superiores e inferiores. Lesões queiloidiformes constituem a manifestação clínica mais comum. A remoção cirúrgica é o procedimento terapêutico de escolha nos casos iniciais, enquanto itraconazol e clofazimina, isolados ou em associação, podem ser utilizados nos casos disseminados. Esta dermatose, típica das áreas tropicais e sub-tropicais, acomete principalmente a pele exposta, poupando as mucosas. As lesões queloidiformes são a apresentação clínica mais comum. Sabe-se que se associa a doenças sistémicas como doenças reumatológicas, doenças inflamatórias intestinais, neoplasias malignas, doenças hematológicas, dentre elas a hemoglobinúria paroxística noturna. Manifesta-se através de lesões cutâneas ulceradas e dolorosas com evolução rápida e progressiva, sendo mais comum nos membros inferiores. As ulcerações podem surgir espontaneamente ou induzidas por traumas, sendo característico do pioderma gangrenoso o fenômeno de patergia. O período entre o início das lesões e o diagnós-tico costuma ser prolongado. O diagnóstico é baseado em evidências clínicas e confirmado com a exclusão das outras etiologias de lesões ulceradas cutâneas. Relatamos um caso de pioderma gangrenoso em membro superior esquerdo com início após evento traumático em paciente com diagnóstico de hemoglobinúria paroxística noturna. Pyoderma Gangrenosum associated with Paroxysmal Nocturnal Haemoglobinuria ABSTRACT -Pyoderma gangrenosum is a chronic neutrophilic dermatosis with no established etiology. It is often associated with systemic diseases such as inflammatory rheumatic diseases, inflammatory bowel diseases, malignant neoplasms and haematological diseases including nocturnal paroxysmal hemoglobinuria. It presents as ulcerated and painful skin lesions with rapid and progressive evolution, mainly in the lower limbs. Ulcerations may arise spontaneously or induced by trauma (pathergy phenomenon). The period between the onset of the lesions and the diagnosis is usually prolonged. The diagnosis is based on clinical data and confirmed after the exclusion of other causes of cutaneous ulcerations. We report a case of Pyoderma gangrenosum in the left upper limb with onset after a traumatic event in a patient with nocturnal paroxysmal hemoglobinuria. As manifestações extra-articulares da artrite reumatoide (AR) são comuns. Os nódulos subcutâneos aparecem em cerca de 8% dos usuários de metotrexato (MTX) com AR. O MTX é a droga de escolha nessa doença e seu uso pode acarretar o aparecimento de efeitos indesejados como a nodulose acelerada (NA). Esta complicação é um importante diagnóstico diferencial em nódulos subcutâneos de evolução rápida em pacientes com AR e em uso de MTX e inibidores de TNF. Seu diagnóstico precoce é fundamental para que seja instalado tratamento adequado tão logo seja possível. Neste relato de caso, os autores descrevem um caso de NA associado ao uso MTX e etanercept em paciente com AR.
