АННОТАЦИЯ Цель. Оценить функциональное состояние при восстановлении эректильной функции у мужчин после лечения рака методом HIFU, по параметрам сердечно-дыхательного синхронизма и их производных. Материалы и методы. Наблюдения были выполнены на 24 пациентах 45-56-ти лет, направленных в связи с повышением уровня простатического специфического антигена на стационарное лечение в урологическое отделение. При их обследовании был поставлен диагноз-рак предстательной железы I-II стадии. У пациентов проводили пробу сердечно-дыхательного синхронизма и по ее параметрам определяли индекс регуляторно-адаптивного статуса и регуляторно-адаптивные возможности. После постановки диагноза пациентам было назначено разрушение опухоли методом трансректальной высокоинтенсивной фокусированной ультразвуковой абляции. После операции больные были выписаны из стационара на следующий день, и дальнейшее наблюдение проводилось в амбулаторном режиме с посещением стационара через 1, 3, 6, 12 месяцев. Через 6 месяцев пробу сердечно-дыхательного синхронизма повторяли. По анкетам-опросникам субъективно оценивали международный индекс эректильной функции МИЭФ-5, по индексу QOL-качество жизни. Статистический анализ результатов исследования был проведен с использованием программ: «STATISTIKA 6,0 for Windows». После установления нормальности распределения вариант использовали достоверные различия в сравнении средних величин, в парных сравнениях брали t-критерий Стьюдента при р<0,05. Результаты. До лечения в группе из 24 пациентов регуляторно-адаптивные возможности оценивались как удовлетворительные. После лечения рака предстательной железы методом трансректальной высокоинтенсивной фокусированной ультразвуковой абляции происходило резкое уменьшение уровня простатического специфического антигена с 6,81±0,32 нг/мл до 0,70±0,01 нг/мл (p<0,001), объема опухоли с 39,8±0,4 см 3 до 8,5±0,3 см 3 (p<0,001). Через 6 месяцев после лечения рака предстательной железы диапазон синхронизации увеличивался на 22,5%. Длительность развития синхронизации уменьшалась на 39,1%. Это приводило к увеличению индекса регуляторно-адаптивного статуса на 100,6%. Регуляторно-адаптивные возможности изменялись от уровня «удовлетворительно» до уровня «хорошо». Через 6 месяцев после лечения рака предстательной железы у больных часть предстательной железы сохранялась, и семяизвержение было возможным. Эректильная функция возобновлялась через 6 месяцев после операции, когда восстанавливались регуляторно-адаптивные возможности. Причем у 16 человек, индекс регуляторно-адаптивного статуса которых составил 77,9±0,4, международный индекс эректильной функции был равен 23,8±0,1 баллов. Это свидетельствует о том, что состояние эрекции соответствует норме. Индекс оценки качества жизни QOL у них составил 0,9±0,1 баллов, что соответствует показателю хорошего качества жизни. У 8 пациентов, у которых индекс регуляторно-адаптивного статуса был 44,6±0,4, международный индекс эректильной функции составил 18,2±0,3 баллов, что указывает на эректильную дисфункцию легкой степени. Описано собственное клиническое наблюдение успешного хирургического лечения первично-множественной синхронной опухоли забрюшинного пространства (гемангиоперицитома) с органосохраняющей резекцией опухоли почки.
