Менингиомы -одни из самых распространенных доброкачественных опухолей головного мозга. При инвазии опухолью синусов головного мозга необходим тщательный выбор тактики лечения. Выбор радикальности вмешательства у этих пациентов является сложной проблемой. Цель исследования -представить собственное клиническое наблюдение радикального удаления менингиомы, инвазирующей в верхний сагиттальный синус с его шунтированием, и обзор литературы по данной проблематике. Материал и методы. В работе представлено клиническое наблюдение пациента 44 лет с парасагиттальной атипической менингиомой средней трети фалькса (Grade II), вызвавшей полную окклюзию верхнего сагиттального синуса (ВСС) (тип VI по Krause). Пациенту выполнено радикальное удаление менингиомы с резекцией участка ВСС и его последующим шунтированием при помощи аутотрансплантата из лучевой артерии. Результаты. В результате хирургического вмешательства радикально удалена анапластическая менингиома, шунтирован ВСС с использованием вставочного трансплантата. 6-месячное наблюдение за пациентом показывает отсутствие рецидива и неврологический дефицит на предоперационном уровне. Заключение. Существует целый ряд вопросов, которые должны быть решены для успешных операций по реконструкции венозной системы: показания к реваскуляризации, выбор трансплантата, использование временного шунта, остановка кровотечения на момент формирования анастомоза, проверка состоятельности шунта.Финансирование. Cardiovascular and oncological diseases are the leading causes of adult death in the world. Despite proven efficacy, anticancer drugs can cause severe cardiovascular complications. Recently, data have appeared on the possible vasotoxic effects of chemotherapy drugs, which can manifest themselves as the progression of arterial hypertension and atherosclerosis, the development of myocardial ischemia and acute coronary syndrome, the formation of venous and arterial thrombosis. The key mechanism for the development of vasotoxicity is endothelial dysfunction, and anticancer drugs can also affect the processes of thrombosis. The review presents the results of 12 selected observational retro- and prospective studies involving cancer patients receiving presumably vasotoxic therapy. Data on the frequency of occurrence and possibilities for the prevention of vasotoxicity are presented.
