Particular features of the personality of patients with vocal cord granuloma have been reported in the literature based on intuitive observation of individual cases in clinical examination. The present study was designed to examine systematically a larger number of patients with vocal cord granuloma by means of a psychosomatic interview and with objective standardised personality inventories. The aim was to investigate characteristics of personality structure, the extent and form of psychosomatic complaints, and the psychosocial self-image of these patients. 33 patients with the diagnosis of vocal cord granuloma participated in the study. They underwent a psychodynamic psychosomatic interview and filled in the Giessen test, a standardized personality inventory, the Giessener Beschwerdebogen, a somatic complaints list, and a social questionnaire. Descriptive statistics were computed. Psychodynamic diagnostics revealed two types of the functional level of personality: narcissistic and psychosomatic structures were almost equally represented. With respect to the drive organisation of personality structure, mainly compulsive and to a lesser extent depressive dominant features were found. The structure of somatic complaints in our patient group with vocal cord granuloma was different from that of the norm population as well as that of a group of psychosomatic-psychoneurotic outpatients. With regard to the extent of somatic complaints, the scores of our patients were in the middle between the norm values and those of psychosomatic patients. In the self-image of their personality and their psychosocial relations, patients with vocal cord granuloma deviated from the values of the norm population in all scales into the direction of the scores of psychosomatic-psychoneurotic outpatients.(ABSTRACT TRUNCATED AT 250 WORDS)
In den letzten 20 Jahren konnte die 5-Jahres-Überlebenszeit bei Patienten mit Kopf-Hals-Plattenepithelkarzinomen trotz verbesserter chirurgischer und radiologischer Techniken kaum gebessert werden. Der Einsatz von Chemotherapeutika als adjuvante oder neoadjuvan-te Therapiemodalität wird weiterhin kontrovers diskutiert und hat in fast alien Studien zu keiner Verlängerung der Überlebenszeit geführt. Die Inzidenz der Plattenepithelkarzinome von Mundhöhle und Pharynx nimmt seit etwa 20 Jahren kontinuierlich zu; derzeit stehen Kopf-Hals-Plattenepithelkarzinome weltweit mit 6% an 6. Stelle aller Krebserkrankungen. Die 5-Jahres-Überlebenszeit kann bei Kopf-Hals-Plattenepithelkarzinomen nach heutigem internationalem Kenntnisstand nur durch ein adäquates adjuvantes tumorimmunkompetentes Behandlungskonzept gebessert werden, da sämtliche Standardtherapieverfahren zusätzlich zu der vorliegenden Immundefizienz immunsupprimierend wirken. Wesentliche Reaktionen innerhalb des inter- und intrazellulären Informationssystems und damit auch des Immunsystems werden durch Lektin-Kohlenhydrat-Interaktionen vermittelt. In Grundla-genversuchen mit Zellkulturen sowie im Tierversuch und beim Menschen konnte gezeigt werden, dass das galaktosidspezifische Mistellektin (ML·1), welches durch Affinitäts- und Gelfiltrationschromatographie gereinigt werden konnte und mit elektrophoretischen Methoden sowie der Gasphasensequenzierung charakterisiert wurde, dosisabhängig immunmodulierend, d.h. immunsupprimierend bzw. -stimulierend wirkt. Als optimale Dosis für die Immunstimulierung wurde eine Dosierung von 1 ng/kg Körpergewicht ermittelt. Seit 1990 ist ein wässriges Extrakt aus der Pappelmistel vom Bundesgesundheitsamt zugelassen (Eurixor®), welches im Gegensatz zu den anderen auf dem Arzneimittelmarkt befindlichen Mistelpräparaten einen normierten Gehalt an Mistellektin ML·1 enthält. In vitro konnte demonstriert werden, dass ML·1 zuckerspezifísch an Lymphozyten und Monozyten bindet, eine erhöhte Phosphorylierung des 28-kD-Proteins (T-Lymphozyten-Rezeptor) und des Phosphatidyl-4,5-Biphosphats bewirkt, einen Anstieg von verfügbarem Kalzium induziert, eine signifïkante Interleukin (IL)-2- bzw. HLA-DQ-Rezeptor-Expression als Korrelat der T- und B-Lymphozyten-Aktivierung induziert, eine starke Zytokinfreisetzung durch mononukleäre Immunzellen hervorruft (u.a. IL·l, IL·2, Interferon γ, Tumornekrosefaktor α) und keinen Einfiuss auf die Proliferationsrate menschlicher und muriner Tumorzellen ausübt. In vivo konnte u.a. in zwei murinen Tumormodellen die Zahl der Lungen- und Lebermetastasen nach subkutaner ML·l-Injektion signifikant reduziert werden. Es existiert bis heute weltweit keine prospektiv angelegte kontrollierte Studie, die die adjuvante Wirkung von ML·l bei einer defínierten Tumorentität auf die entscheidenden Kriterien in der Onkologie wie Einfiuss auf Überlebenszeit, Rezidivhäufigkeit oder die Lebensqualität evaluiert. Zusammenfassung Fallberichte zur präklinischen Durchführung einer Clamshell-Thorakotomie in Deutschland sind bisher rar, wenngleich diese Notfallprozedur in internationalen Leitlinien zur Behandlung des traumatischen Kreislaufstillstands enthalten ist. Im vorliegenden Beitrag wird die Versorgung eines erwachsenen Patienten nach einem Messerstich in den als „cardiac box“ bezeichneten Bereich des Thorax dargestellt. Konventionelle Maßnahmen der kardiopulmonalen Reanimation wie Atemwegssicherung, Beatmung und Thoraxkompression führten nicht zur Wiederherstellung des Kreislaufs („return of spontaneous circulation“, ROSC). Nachdem auch nach Ausschluss eines Spannungspneumothorax mithilfe einer Minithorakotomie kein dauerhafter ROSC erzeugt werden konnte, wurde entsprechend der Leitlinienempfehlung eine Thorakotomie erwogen und vor Ort durchgeführt. Nach dem Ausräumen einer Perikardtamponade und anschließendem ROSC wurde eine stark blutende Myokardverletzung übernäht und der Patient in den Schockraum des Universitätsklinikum Heidelberg gebracht. Dort erfolgten eine Notfalltransfusion und konsekutiv die definitive Versorgung der Verletzung im kardiochirurgischen OP. Der Patient verstarb am Folgetag an den Folgen eines hypoxischen Hirnödems im Beisein seiner Angehörigen.
