Aim To asses if telemedicine with telemonitoring is a clinically useful and secure tool in the tracking of patients with COVID-19. Methods A prospective observational study of patients with COVID-19 diagnosis by positive PCR considered high-risk tracked with telemedicine and telemonitoring was conducted in the sanitary area of Lugo between March 17th and April 17th, 2020. Two groups of patients were included: Outpatient Tracing from the beginning and after discharge. Every patient sent a daily clinical questionnaire with temperature and oxygen saturation 3 times a day. Proactive monitoring was done by getting in touch with every patient at least 11 a day. Results 313 patients (52.4% female) with a total average age of 60.9 (DE 15.9) years were included. Additionally, 2 patients refused to join the program. Since the beginning, 224 were traced outpatient and 89 after being discharged. Among the first category, 38 (16.90%) were referred to Emergency department on 43 occasions; 18 were hospitalized (8.03%), and 2 deceased. Neither deaths nor a matter of vital emergency occurred at home. When including patients after admissions monitoring was done in 304 cases. One patient re-entered (0.32%) to the hospital, and another one left the program (0.32%). The average time of monitoring was 11.64 (SD 3.58) days, and 224 (73.68%) patients were discharged during the 30 days of study. Conclusions Our study suggests that telemedicine with home telemonitoring, used proactively, allows for monitoring high-risk patients with COVID-19 in a clinically useful and secure way.
RESUMEN Varón de 68 años de edad, diagnosticado de adenocarcinoma de pulmón y mixoma auricular derecho, al realizar una radiografía de tórax en un preoperatorio por enfermedad de Dupuytren bilateral. El interés de este caso vendría, por su diagnostico diferencial, por su manejo, por la posible asociación entre ambos tumores (1), y por la escasez de publicaciones encontradas.
Los tumores carcinoides tienen su origen en las células de Kulchitsky. Estas células derivan de la cresta neural y se encuentran distribuidas por todo el organismo. La incidencia de estos tumores es muy baja, estimándose en nuestro país en 7 casos por millón de habitantes y año. La localización más frecuente es en el tubo digestivo. La mitad de todos los tumores carcinoides asientan en la región apendicular (1). Presentamos el caso de un paciente con manifestación atípica tanto clínica como radiológica de un tumor carcinoide bronquial.Varón de 38 años de edad, electricista, con antecedentes de varios episodios de prostatitis e hiperuricemia asintomática. Durante los cinco meses previos a su ingreso presentó episodios febriles de repetición sin afectación del estado general ni otra clí-nica acompañante, que fueron diagnosticados por su médico de centro de salud de neumonía basa izquierda en base a los hallazgos clínicos y en la radiografia de tórax. Se trataron con antibioterapia empírica con resolución de los mismos. Ingresa en mayo de 1998 por sensación distérmica, temperatura de 38ºC y dolor en hemitórax izquierdo no irradiado. En la exploración física se objetiva un paciente eupneico con estado general conservado, normocoloreado y normoperfundido. En la auscultación pulmonar presenta hipoventilación en hemitórax izquierdo sin signos de condensación. La auscultación cardiaca fue normal. El hemograma puso de manifiesto una leucocitosis ligera con fórmula normal. La bioquímica de ingreso, coagulación básica, proteinograma e inmunoglobulinas, velocidad de sedimentación globular en la primera hora, analítica básica y sedimento urinario fueron normales. La radiografia posteroanterior y lateral de tórax (Fig. 1) mostraba un aumento de densidad homogéneo en región anterior y 1/3 inferior y medio de hemitórax izquierdo sin broncograma aéreo, bien delimitado y en continuidad con borde paracardial izquierdo; signos compatibles con aumento de la arteria pulmonar izquierda (estos hallazgos eran similares a los que presentaba en las radiografías de tórax realizadas en los meses previos). El ecocardiograma fue informado como normal. Se realizó TAC torácico que mostraba un colapso del segmento anterior del lóbu-lo superior izquierdo, con adenopatías mediastínicas menores de un centímetro. El TAC abdominal evidenció un ligero aumento de la suprarrenal izquierda. Se realizó fibrobroncoscopia observándose una oclusión completa del LSI por tumoración endobronquial muy vascularizada que se biopsió. La anatomía patoló-gica fue compatible con tumor carcinoide típico. Los valores de cortisol plasmático fueron normales. La determinación de serotonina, 5-Hidroxitriftófano y 5-Hidroxiindolacético en orina de 24 horas fueron normales. Los hemocultivos fueron negativos.Al ingreso se instauró tratamiento con antibioterapia empírica quedando el paciente afebril en las primeras 48 horas y manteniendo un buen estado general durante los días posteriores.
