Пирофосфатная артропатия (ПФА) -заболевание, яв-ляющееся результатом образования и депонирования кри-сталлов пирофосфата кальция (ПФК) в суставах и развиваю-щегося в связи с этим воспаления иммунного характера. Яв-ляясь микрокристаллическим артритом, ПФА, тем не менее, по патогенезу ближе к другим кальцифицирующим болезням, чем к подагре. Показана общность ингибиторов/промоуте-ров кальцификации и генетических детерминант многих кальцифицирующих болезней, в том числе артритов, свя-занных с кристаллами кальция (пирофосфатами и основны-ми кристаллами кальция), кальцификации при атеросклеро-зе (как в области интимы, так и в области медии) [1].Три инновационных для своего времени метода явились ключевыми для открытия ПФА: с внедрением рентгенографии был описан феномен «хондрокальциноза»; с появлением по-ляризационной микроскопии -неуратные кристаллы в сино-виальной жидкости, которые при рентгеновской дифракции были расценены как пирофосфат кальция дигидрата (ПФК, Ca2 P2O7 ⋅ 2H2O) [2]. Интерес к ПФА был особенно высок в 80-е годы прошлого века, что связано с подробным изучением и описанием клинических симптомов заболевания. Однако по-степенно он угас, чему способствовали 3 причины: 1) кажущаяся малочисленность популяции больных ПФА и ложное отнесение ее исключительно к старческому возрасту;2) сложность визуализации кристаллов ПФК из-за сла-бого лучепреломления, что затрудняет диагностику ПФА с помощью относительно дешевого метода -поляризацион-ной микроскопии;3) отсутствие патогенетической терапии -препараты, растворяющие депозиты кальция, пока не созданы.В связи с этим диагностика ПФА не проводится активно, а кристаллы ПФК чаще являются находкой при артроскопии, морфологическом исследовании структур сустава с целью дифференциальной диагностики, а также резецированного материала при артропластике или секционного материала.

Изучить частоту выявления артериальной гипертензии у больных подагрой с помощью суточного мониторирования артериального давления (СМАД). Результаты. В исследование включено 93 больных подагрой, которые были условно разделены на две группы. В первую (I) вошли пациенты с нормальными показателями артериального давления (АД) при его измерении по методу Короткова-22 человека (24%), во вторую (II)-пациенты с повышенными цифрами АД-71 человек (76%). Всем больным проведено суточное мониторирование АД. У 100% больных в обеих группах выявлены различные нарушения параметров СМАД (увеличение среднесуточного АД, пульсового давления, индексов нагрузки давлением, нарушение суточного профиля АД), что свидетельствует о наличии скрытой АГ у больных подагрой из I группы. Выводы. Выявление нарушений показателей СМАД в группе больных подагрой с нормальными значениями АД при его измерении по методу Короткова указывает на наличие скрытой АГ у этих пациентов.
BackgroundMonitoring of RA activity is important in daily practice and clinical trials, in particular for subjects receiving biologics. As a result, new imaging tools and more objective biomarkers of synovial inflammation and joint destruction have been developed.ObjectivesTo evaluate role of imaging methods (joint MRI, US), biomarkers (calprotectin-CP and MMP-3) and RA activity parameters in achievement of ACR70 response after Etanercept (ETN) therapyMethods25 pts (mean age 49,4±13,7 yrs) with active RA despite stable doses of cDMARDs (DAS28 6,12±0,97) had received ETN 50 mg weekly sc for 24 weeks. RA duration was 6,38±5,9 yrs, 92% were women, 64%>RF+. Clinical and laboratory evaluation was done at weeks 0 and 25. US exam was performed before start of ETN treatment and after 6 month of ETN therapy by the same independent sonographer. 7-joint score (grade 0–3) included wrist, intercarpal, 2–3 MCP, 2–3 PIP joints of the clinically dominant hand and knee was used. MRI was done at baseline and after end of ETN treatment. Assessment of 2–5 MCP joints of the clinically dominant hand (0-no synovitis, 1–10-severity of synovitis) was done by the same independent radiologist.CP and MMP-3 serum levels (ELISA) were measured at the same time points. Changes of all parameters since baseline were assessed. Predictive factors of achievement ACR70 responce were explored.ResultsAll clinical, laboratory, MRI and US parameters of RA activity had significantly improved during ETN therapy (p<0,05).We detected statistically significant difference only in baseline MRI synovitis score between responders and non-responders according ACR70 (p=0,0074). US and MRI synovitis sum scores decreased significantly in parallel with CP and MMP-3 serum levels and RA activity variables during ETN therapy. 2. Low baseline MRI synovitis sum score is more useful in comparison with baseline US, CP, MMP-3 and conventional RA activity factors as predictive tool of good responce in RA during anti-TNF therapy. 3.
