Background. The silver nanoparticles (Ag NPs) are the most acceptable and excellent nanotechnology-based product among all metallic nanoparticles (noble metals). They are novel in terms of good conductivity, catalytic, chemical stability, and most significant anti-inflammatory and antiviral activities. Aim. This review is aimed at understanding the synthesis, mechanism, and applications of Ag NPs in dentistry and their qualities to aid clinicians and researchers. Materials and Methods. A electronic search for literature was performed on the Google Scholar, PubMed, EMBASE, and Web of Science databases for related articles using multiple keywords. The keywords used were antimicrobial properties, mechanism of action, and current uses of Ag NPs in dentistry. The search was limited to articles published in the English language; no deadline was set for publications. Results. About 185 articles were considered relevant to the reported research out of 6420 electronic search results. Only the publications that were required were received after all abstracts were assessed for relevance. After a final electronic and manual search, roughly 47 research publications were found to be helpful in this study. Conclusion. The Ag NPs, unlike other biomaterials used in dentistry, have unique biological features and could be used in endodontics, restorative dentistry, periodontology, prosthetic dentistry, implantology, and oral malignancies, because of their antimicrobial, antifungal, and antiviral effects.
Objective: Endodontic treatment should be both conservative and effective. Endodontic instruments with a greater taper are used for coronal flaring, for proper debridement with efficient irrigation. However, increased taper of an instrument can remove a larger amount of pericervical dentin, compromising the strength of the tooth. The aim of this study was to determine the effect of hand files, ProTaper Universal, ProTaper Next, and V Taper rotary instrument systems on the fracture resistance of teeth. Materials and Methods: In total, 60 extracted human maxillary first premolars were divided into four groups—Group I (Hand Files; HF), Group II (ProTaper Universal; PT), group III (ProTaper Next; PTN) and Group IV (V Taper; VT) (N = 15). Each group was instrumented with the respective instrument system, irrigated, obturated, restored, and mounted in cold cure acrylic. A universal load-testing machine (Shimadzu, Japan) was used to apply a vertical compressive load. The maximum force was recorded in Newton. Analysis of variance (ANOVA) and Independent t-tests were applied to compare the maximum mean force required to fracture the tooth. Results: There was a statistically significant difference in fracture resistance between Group I (HF) and Group II (PT) and between Group II (PT) and Group IV (VT) (p < 0.001). Similarly, a significant difference was observed between Group II (PT) and Group III (PTN) (p < 0.01). Furthermore, a significant difference was observed between Group I (HF) and Group III (PTN), and between Group III (PTN) and Group IV (VT) (p < 0.05), too. However, there was no statistically significant difference between Group I (HF) and group IV (VT) (p > 0.05). Conclusion: Rotary files with more taper seem to remove more pericervical dentin than traditional manual and rotary files with less taper, thus altering the strength of the tooth.
Introduction: Permanent mandibular first and second molars may display extra roots namely radix entomolaris and radix paramolaris which may have implications in endodontic treatment outcome, if missed. Objective: To evaluate the occurrence of extra roots in permanent mandibular first and second molars in a sample of Nepalese population. Methods: This analytical cross-sectional study was done at Dhulikhel hospital. Convenience sampling technique was utilised for data collection of 773 CBCT images. Images from June 2018 to June 2020 were retrospectively screened for presence of fully erupted bilateral mandibular first and second molars. Presence of extra roots were recorded and laterality, gender, and racial variations were analysed by Fisher’s exact test and Chi-square test using SPSS v.20. Results: For mandibular first molars, out of 517 patients, 65 (11.38%) had radix entomolaris: 38 (13.2%) female and 27 (9.54%) male. Among 38 females; occurrence was 21 (7.3%) bilateral, 16 (5.56%) unilateral right and 1 (0.34%) unilateral left side. Likewise, among 27 males, the occurrence was 15 (5.3%) bilateral, 6 (2.1%) unilateral right and 6 (2.1%) unilateral left side. Regarding races, 50 (14.6%) were Mongoloids and 15 (6.6%) were Aryans. No radix paramolaris was found in mandibular first molars. For mandibular second molars, out of 623 patients, radix entomolaris and paramolaris were observed in 0.8% and 0.48% respectively. Conclusion: The overall occurrence of radix entomolaris in mandibular first and second molars was found to be 11.38% and 0.8%, respectively. Practitioners should be aware of these unusual variations to avoid iatrogenic mishap due to missed canal.
Background Pulp stones or “denticles” are a frequent finding in coronal and radicular pulp of primary and permanent dentition. These discrete calcified bodies can be seen in a healthy, diseased and sometimes even in an unerupted tooth. These calcifications are mostly present in molars as free, attached or embedded to dentine of pulp chamber or root canals. Objective To determine the occurrence of pulp stones in molars of selected adult Nepalese population using radiographs and assess the association of its occurrence gender and arch wise. Method A descriptive cross-sectional study was performed using orthopantomogram of total 380 patients (190 male and 190 females) of age group ranging from 16-30 years by convenience sampling. Data were collected through the examination of both hard and soft copies of radiographs under magnification. Pulp stones were scored as present or absent and their association with gender and dental arch were recorded. Descriptive statistics and chi square test was performed to see the prevalence and to compare the occurrence of pulp stone respectively. Result Out of 380 radiographic samples, pulp stones were present in 41.05% of the samples. The occurrence of pulp stones were found to be higher in females than in males, although the difference was not statistically significant (p=0.060).The total number of teeth with pulp stones was 238 and 61 in maxillary and mandibular arches respectively, with statistically significant difference (p<0.001). Conclusion The overall occurrence of pulp stone was found to be 41.05%. It was observed more in females (45.78%) than in males (37%) and more frequently located in maxillary molars (29.2%) than in mandibular molars (8.2%).
