INTRODUCCIÓNLos cambios demográficos producidos en España en los últimos años son similares al resto de los países desarrollados de nuestro entorno, con un incremento de la proporción de personas mayores y un descenso del porcentaje de jóvenes (1). El censo de 2001 (1), estimaba la población española mayor de 65 años en 6.964.267 individuos, que representan el 17% del total de los españoles.Estas características demográficas han condicionado que en la clínica diaria exista un aumento progresivo de pacientes en edades avanzadas (2-4) y por tanto enfermedades crónicas asociadas (2,4).Siguiendo la misma línea, las proyecciones demográfi- [0212-7199 (2006) RESUMENObjetivos: Estudiar las características clínico epidemiológicas de los pacientes ingresados en un servicio de Medicina Interna.Material y métodos: Estudio descriptivo trasversal, en el que se incluyeron los pacientes mayores de 65 años que fueron ingresados a lo largo del año 2002 en un Servicio de Medicina Interna. Las variables recogidas para cada paciente fueron: edad, sexo, tiempo de ingreso, patologías crónicas anteriormente diagnosticadas, factores de riesgo cardiovascular, motivo de ingreso hospitalario (síntoma guía por el que el paciente decide acudir al centro hospitalario), exitus intrahospitalario, diagnósticos clínicos al alta y tratamientos prescritos. Se emplearon índices estadísticos descriptivos de variables cualitativas y cuantitativas, Chi-cuadrado para variables cualitativas y Mann-Withney-Wilcoxon para variables cuantitativas.Resultados: Se incluyeron un total de 770 pacientes. La proporción entre sexos es similar, aunque las mujeres presentan mayor mediana de edad (p < 0,001). La patología más prevalente entre los antecedentes personales fue la Cardiopatía isquémico-hipertensiva (25,7%) y el 64,9% de los pacientes presentaron algún factor de riesgo cardiovascular. Los varones presentaban mayor número de factores de riesgo cardiovascular asociados y mayor número de patologías (p < 0,01 en ambos casos). La disnea fue el motivo de ingreso más frecuente (42,7%). El diagnóstico más frecuente fue la insuficiencia cardiaca (20,6%). Se produjo el fallecimiento en el 16,6% de los pacientes, como principal diagnóstico presentaron la infección respiratoria (49,5%).Conclusiones: La patología más prevalente y que causa más ingresos son las enfermedades crónicas de los países desarrollados. La prevención primaria y secundaria es la forma más eficaz de control de estas patologías.
A retrospective study of 80 HIV patients diagnosed with tuberculosis was carried out in order to evaluate the clinical presentation of tuberculosis (CPT) in relation to the degree of HIV-induced immunodeficiency, as determined by the CD4 lymphocyte count and reactivity to the tuberculin and delayed-type hypersensitivity reaction (DHR) skin tests by applying 2TU of RT-23 tuberculin, and the Muiltest IMC Merieux, respectively. CPT classification was undertaken on the basis of the location of the disease: pulmonary tuberculosis (PT), distinguishing between typical pulmonary (TP) and atypical (AP) according to the radiological pattern; extrapulmonary (ET); mixed tuberculosis (MT) pulmonary and extrapulmonary; and miliary tuberculosis. The CD4 lymphocyte count was 264.6 +/- 226.8, the TP had the highest number (505), MT had 132 (p < 0.001) and the miliary tuberculosis had 148 (p < 0.001), the lowest. The tuberculin skin test was positive in 35%, of which 11% were MT (p < 0.05). In the delayed-type hypersensitivity reaction, 67% were non-normoergic, of which 95% and 100% were MT and miliary tuberculosis, respectively (p < 0.05). There was a good overlap between CD4 depletion and skin tests. TP exhibited moderate immunodeficiency, AP severe immunodeficiency, and mixed and miliary TB extremely high immunodeficiency.
Desde el inicio de la pandemia por la infección por el virus de la inmunodeficiencia humana (VIH) hemos podido asistir a un incremento en las cifras de casos nuevos de infección a lo largo de todas las zonas en que ambos gérmenes coexisten (1-5) que cada vez mayor al aumentar de la esperanza de vida por la mejora en el tratamiento antiretrovírico (6). Este aumento de incidencia de la tuberculosis se objetiva tanto en zonas endémicas y países subdesarrollados como en aquellos estados en que la tuberculosis se consideraba una curiosidad perteneciente a tiempos pasados (7-9). Es en estos últimos países donde formas atipicas y enfermedad causada por gér-menes multirresistentes han causado problemas de salud seve- [0212-7199(2000) RESUMENObjetivo: El objeto de este trabajo fue determinar la inmunidad celular utilizando el Multitest CMI y correlacionar su resultado con el recuento de linfocitos CD4, y el riesgo de desarrollar tuberculosis activa en el caso de que la pápula correspondiente a la tuberculosis del multitest fuera positiva.Material y Métodos: Se realizó un seguimiento longitudinal prospectivo de 342 pacientes UDVP, 210 infectados con el VIH-1 y 132 de ellos seronegativos. La cohorte objeto del estudio estaba formada por 165 UDVP seropositivos para el VIH-1 (128 varones y 37 mujeres). Las edades estaban comprendidas entre 18 y 45 años. El tiempo del trabajo fue de 25 meses (8-25). La prueba de la tuberculina y el multitest les fueron practicados al inicio y cada tres meses así como un despistaje de tuberculosis activa a lo largo de todo el periodo del estudio cuando se consideró necesario.Resultados: Se desarrolló tuberculosis activa en 9,25 casos por cien personas y año de seguimiento en los estados con mejor nivel inmunoló-gico (score de la PHR >10 mm).Había una correlación significativa entre el diámetro de la pápula de la prueba de la tuberculina y la pápula de la fracción tuberculínica de la prueba de hipersensibilidad retardada.Conclusiones: La prueba de la hipersensibilidad retardada realizada con el Multitest IMC es de utilidad para evaluar la función de la inmunidad celular y en los pacientes seropositivos para el VIH-1 anergicos o con la fracción tuberculinica de la PHR positiva debe considerarse la posibilidad de quimioprofilaxis con isoniazida (9 meses).
An elderly patient with a bronchoesophageal fistula secondary to tuberculosis and a 3-month history of cough and dysphagia worsening with deglutition was admitted to hospital. Radiological examination and CT of the thorax revealed mediastinal adenopathy. Bronchoscopy, esophagoscopy and esophagraphy confirmed the presence of a bronchoesophageal fistula. Histological examination of the esophagus and bronchial biopsy specimens revealed nonnecrotic granulomas, and the acid-fast bacilli tested positive for Mycobacterium tuberculosis in respiratory secretions. Antituberculous treatment was started after diagnosis of intrathoracic lymph node tuberculosis with bronchoesophageal fistulization. A month and a half after initiating treatment, paroxistic coughing during deglutition persisted. An esophagoscopy was performed, and the orifice was closed with a fibrin tissue. After 9 months of treatment, the patient was asymptomatic and in good health.
The prevalence cardiac and pulmonary disease prevalence is high, these diseases are the of the most frequent causes hospital mortality.
