BACKGROUND: Myotonic dystrophy (MD) is characterized by myotonia, multisystemic lesions and hypogonadism.In women, the relationship between MD and infertility remains controversial. This study investigated the ovarian status and response to controlled ovarian stimulation (COS) in MD women entering our preimplantation genetic diagnosis programme. METHODS: We elected to compare MD patients with X-linked disorders (XLD) carriers, given that XLD have not been shown to affect ovarian status. On the one hand, we analysed all the cycles performed and, on the other hand, we conducted a subanalysis based on only first cycles. RESULTS: MD and XLD groups were similar with regard to women's ages, day 3 parameters, number of oocytes retrieved, embryos obtained and prevalence of top quality embryos. The day of HCG was significantly delayed and the prevalence of poor quality embryos was higher in the MD group. The subanalysis on first cycles only also showed significantly fewer mature follicles on the day of HCG in MD population. Implantation and pregnancy rates were similar in both groups; however, no pregnancy occurred at the first cycle in MD (0 out of 4), whereas 77% of pregnancies (10/13) occurred at the first attempt in XLD carriers. CONCLUSIONS: These results indicate that the responsiveness to COS was moderately hindered in MD women as compared to controls. Reassuring data about implantation and pregnancy rates support the feasibility of PGD in selected mildly affected MD women.

Depuis mars 2020, la Tunisie est exposée à la crise sanitaire de la pandémie COVID-19. Son impact sur la santé mentale a été attesté par plusieurs études avec un sur-risque de survenue de détresse psychologique chez les patients chroniques.

Il s’agit d’une étude transversale menée au service d’hémodialyse de la polyclinique de la CNSS, Sfax, Tunisie, au mois d’avril 2021, date de l’installation de la 4
e
vague COVID-19. Nous avons proposé à nos patients de participer en répondant à un questionnaire d’une vingtaine de minutes.
Méthodes
Le questionnaire a été développé en se basant sur l’Impact of Event Scale-Revised (IES-R), afin de mesurer le stress post-traumatique. Nous avons aussi choisi cinq questions du General Health Questionnaire (GHQ), pour déterminer la prévalence de la détresse psychologique sur les domaines de la dépression, l’anxiété et la désadaptation sociale. Des questions supplémentaires ont été ajoutées afin de documenter les difficultés propres à chaque patient.
Résultats
Trente et uns des 45 patients du service ont accepté le questionnaire, soit un taux de participation de 68,8 %. Les données générales sont résumées dans le Tableau 1. La plus grande crainte de nos patients était qu’ils soient infectés par le COVID-19 (64,5 %), ou qu’un membre de leurs familles soit infecté (67 %). La majorité ne ressentent pas d’influence particulière de la pandémie sur la sévérité de leur maladie chronique (84 %). Les questions du GHQ ont montré des taux élevés d’anxiété (45 %), de perte de sommeil (39 %), de dépression (48 %) et de désadaptation sociale (74 %). Onze patients ont eu un score IES supérieur ou égal à 36, Soit un stress post-traumatique présents chez 30,5 % des répondants.
Conclusion
L’assistance psychologique et le dépistage d’une souffrance psychologique particulière sont obligatoires pour minimiser les dégâts de cette pandémie sur la santé mentale des hémodialysés.
