гАуз ко «областная детская клиническая больница», г. кемерово, россия 2 Фгбу «нии комплексных проблем сердечно-сосудистых заболеваний», г. кемерово, россия Головной мозг представлен тканью с высокой метаболической активностью, и его повреждение приводит к нарушению снабжения кислородом и нутриентами, сопровождающемуся тяжелым жизнеугрожающим состоянием-отеком головного мозга� Отек головного мозга протекает через несколько стадий, каждая из которых имеет уникальные патогенетические механизмы� Стадия цитотоксического отека характеризуется перераспределением жидкости во внутриклеточное пространство� Стадия ионного отека характеризуется функциональным нарушением гематоэнцефалического барьера с перераспределением жидкости в интерстиций� Стадии вазогенного отека и геморрагического преобразования характеризуются анатомическим повреждением гематоэнцефалического барьера� Традиционные методы лечения отека головного мозга, такие как применение диуретиков, гиперосмолярных растворов, гипервентиляции, декомпрессионной краниотомии, не показали эффективности� Современные данные о патофизиологии отека мозга способны открыть перспективные направления его лечения�
гауз ко «областная детская клиническая больница», г. кемерово, россия 2 фгбу «нии комплексных проблем сердечно-сосудистых заболеваний», г. кемерово, россия В статье приведены современные представления о физиологии церебрального жидкостного обмена и патофизиологии отека головного мозга� Первая часть статьи была посвящена описанию механизмов обмена жидкости и электролитов в норме, во второй части дано описание патофизиологических аспектов развития отека головного мозга, протекающего в несколько стадий� Первой стадией является цитотоксический отек, представляющий собой форму перераспределения жидкости между пространствами� Возникающий вслед за этим дефицит натрия в интерстиции приводит к развитию второй стадии-ионного отека, характеризующегося только функциональным нарушением гематоэнцефалического барьера� Последующее анатомическое нарушение гематоэнцефалического барьера приводит к развитию вазогенного отека и переходу к стадии геморрагического преобразования� Понимание механизмов развития отека головного мозга открывает новые перспективы для коррекции данного состояния� Ключевые слова: отек головного мозга, молекулярные механизмы, лечение отека головного мозга для цитирования: Задворнов А� А�, Голомидов А� В�, Григорьев Е� В� Клиническая патофизиология отека головного мозга (часть 2) // Вестник анестезиологии и реаниматологии�-2017�-Т� 14, № 4�-С� 52-60�

Кемерово Тяжелая церебральная ишемия новорожденных остается основной причиной детской инвалидизации и смертности, основным способом снижения которых является применение нейропротекции. Широко внедряемая в настоящий момент в клиническую практику терапевтическая гипотермия обладает рядом ограничений. Это требует поиска эффективных фармакологических нейропротекторов, воздействующих на широкий спектр патогенетических механизмов нейронального повреждения. Такими перспективными нейропротекторами являются мелатонин, эритропоэтин, топирамат, каннабиноиды, барбитураты и магния сульфат. Перспективными направлениями нейропротекции у новорожденных могут быть антенатальное применение при дистрессе плода или потенцирование и/или отсрочка применения терапевтической гипотермии при их постнатальном введении. Имеющиеся на текущий момент данные клинических исследований не позволяют рекомендовать какой-либо из этих препаратов для рутинного клинического применения. Однако проводимые в настоящее время клинические исследования более высокого уровня могут позволить в перспективе выявить эффективные нейропротекторы и оптимальный режим их применения у новорожденных с тяжелой церебральной ишемией.
