Цель исследования-определить показания к проведению консервативной терапии гормональными препаратами при лечении узловой и диффузной форм аденомиоза на основании изучения экспрессии эстрогеновых и прогестероновых рецепторов. Материал и методы. Обследованы 48 пациенток репродуктивного возраста с аденомиозом. Всем пациенткам проводили общеклиническое обследование, инструментальное и лабораторное исследования с целью верификации диагноза. Всем пациенткам было проведено хирургическое лечение. В обеих группах женщины получали гормональную терапию по поводу эндометриоза, которая была отменена за 6 мес перед оперативным лечением. Проводилось гистологическое и иммуногистохимическое исследования операционного материала. Изучалась экспрессия эстрогеновых и прогестероновых рецепторов в эпителии, строме гетеротопий и прилежащем миометрии. Для иммуногистохимического анализа использовались мышиные моноклональные антитела к эстрогеновым рецепторам α (клон 1D5 DAKO, США; разведение 1:35) и прогестероновым рецепторам (клон 636 DAKO, США; разведение 1:50). Результаты исследования. Полученные нами данные свидетельствуют о том, что жалобы пациенток с аденомиозом очень неспецифичны и чаще всего представлены нарушениями менструального цикла. Средняя длительность течения заболеваний у обследованных пациенток значительно различалась: в группе женщин с узловой формой аденомиоза она составила 3,4±1,2 года, в группе женщин с диффузной формой-6,2±1,1 года (p<0,05). Эффективность различных групп препаратов при консервативной терапии была сопоставима. Учитывая отсутствие эффекта от консервативной терапии, все пациентки были оперированы. По данным иммуногистохимического исследования, узловая форма аденомиоза является более гормональнозависимой, чем диффузная. Выводы. Изучение экспрессии эстрогеновых и прогестероновых рецепторов показало, что хотя узловая форма аденомиоза имеет более высокий гормональный статус, чем диффузная, данное различие не влияет на эффективность гормональной терапии. При подтверждении аденомиоза возможно проведение консервативной терапии гормональными препаратами и в случае отсутствия эффекта-оперативное лечение. The purpose of the study was to investigate clinical, morphological and immunohistochemical features and pathogenetic interrelationship between focal, nodular and diffuse forms of adenomyosis. There was investigated surgical samples of removed uterines and their fragments; patients were divided into 3 groups, the 1st group was consisted of 16 cases with focal adenomyosis, the 2nd - 24 females with nodular form of adenomyosis, the 3rd - 21 female with diffuse form of adenomyosis. All patients were underwent general clinical examination, instrumental and laboratory observation with the aim of the verification of the diagnosis. There were executed macro- and microscopic immunohistochemical investigations of fragments of the tissue from the zone of endometrioid heterotopias, surrounding myometrium and eutopic endometrium. There were used antibodies: Ki-67, PTEN, ER, PR (“DAKO”), p16 (“Roshe”). There was established the significant increase in the expression of proliferation marker Ki-67, in parallel with a decrease in the expression level of PTEN, ER, PR from the focal to the nodular and diffuse forms of adenonomyosis. Thus it seems to be possible to consider focal, nodular and diffuse forms of adenomyosis as forms-phases of adenomyosis, that according both to histological characteristics of adenomyosis and immunohistochemical markers offocal and nodular forms may antedate the diffuse form of adenomyosis, that is also confirmed by the detection of a combination of all of these forms. Diffuse form of adenomyosis can be assumed to have the highest potential for the growth and dissemination of the pathological process in the uterine.
