The development of an effective unified information space of the regional radiology diagnostics service is possible by connecting all outpatient cancer care centers and medical organizations to distributed medical image storage systems under the management of regional radiological information systems within the framework of the regional programs “Creation of the unified digital circuit in healthcare based on the unified state health information system” of the territorial subjects of the Russian Federation. The aim of the research is to study the features of using radiological information systems in the oncological profile and develop recommendations for creating distributed archives of medical images in the oncology service within the framework of the federal project. The paper considers the results of the implementation of software products “LINS LookInside”, “LINS Makhaon” in the framework of implementation of local and regional projects in cancer clinics. The data on the connected diagnostic equipment and integration with medical information systems are provided. It has been established that each medical organization with an oncological profile should have its own local PACS installed. Using the example of federal cancer centers, it is recommended to include endoscopic departments, radionuclide and ultrasound diagnostics rooms in radiological information systems, as well as to install and connect systems for automatic publication of CD/DVD discs with DICOM studies.
Введение. Специфика онкологической помощи населению предполагает мультидисциплинарный подход к диагностике и лечению пациентов. Сложная система взаимодействия различных субъектов в рамках организации онкологической службы обуславливает необходимость применения современных управленческих технологий, направленных на улучшение качества и скорости обслуживания пациентов за счет оптимизации процессов. Цель исследования-разработка рекомендаций по маршрутизации пациентов на основе изучения особенностей информационного взаимодействия между всеми уровнями онкологической службы, применения процессного подхода и электронных сервисов. Выявлены причины отсутствия единого информационного пространства в онкологической службе региона. Предложены мероприятия, направленные на оптимизацию маршрутизации пациентов в рамках создания единого цифрового контура в онкологической службе. Методы. Для эффективного реформирования онкологической службы применены методы процессного управления, заключающиеся в создании горизонтальных связей в медицинских организациях, когда задействованные в одном процессе подразделения и сотрудники самостоятельно, без участия вышестоящего руководства, координируют работу и решают возникающие проблемы; методы функционального и информационного моделирования электронных сервисов. Методами статистической обработки данных проведен анализ контингента пациентов со злокачественными новообразованиями. Результаты. Реализована возможность использования электронных сервисов: управления потоками пациентов по направлениям на специализированную помощь, телемедицины, телерадиологии. Создана комплексная информационная система в составе медицинской, лабораторной и радиологической информационных подсистем, интегрированных с электронными сервисами регионального сегмента единой государствен-4.0
Aim research: to develop and implement a patient registration register after relaparotomies in operation on the basis of a clinical oncological dispensary. The premise of this study was the need to obtain statistical data to analyze the effectiveness of vacuum therapy during relaparotomies. By automating the process of recording relaparotomy data, it will be possible to analyze the impact of various factors on patient mortality. Designed the interface and database structure. Based on the described methods and tools, a specialized module of the medical information system for recording relaparotomies has been developed. Since the development took into account all the requirements of medical specialists in a specific field of knowledge, it was possible to develop a specialized software tool that can be used to obtain specific statistics. A patient registration register after relaparotomy was developed and implemented as a specialized module of the medical information system. The register collects, analyzes and interprets data on surgical methods used, including the use of vacuum therapy. To achieve the goal set in this work, the methods and tools presented in the article were used, which were applied in practice and can be similarly used in other medical institutions.
