ÂñòóïНа депресію хворіють 300 млн. людей, і зазначений показник у всьому світі, за даними експертів ВООЗ, щорічно зростає. Хворі на депресію є найбільшою групою ризику виникнення аутоагресивних проявів (АП) та самогубства [12]. Клінічні ознаки АП -суїцидальна активність, самоушкодження, харчова або хімічна залежність [4]. Близько 800 тис. людей щороку позбавляє себе життя, і причина майже половини з них -депресивні розлади, які вийшли на третє місце серед причин смерті [12]. Депресією супроводжуються найбільш поширені психічні та соматичні, в тому числі, онкологічні захворювання, що призводить до серйозних соціальних наслідків -зниження якості життя, сімейної та професійної дезадаптації, додаткових матеріальних витрат в сім'ях хворого та в усьому суспільстві [1]. Етіологія та патогенез депресії вияснено недостатньо. Виділяють легку, середнього ступеня та важку; ендогенну, екзогенну та психогенну депресію; за рівнем порушень -психотичну та непсихотичну [3]. Від депресії, за певних обставин, ніхто не застрахований. Але самотні та знедолені, хворі на рак, інваліди, люди похилого віку, особи, які зазнали екстремальних впливів, та деякі інші, складають найбільшу групу ризику виникнення депресії [3, 4] .Хворі на депресію виявляють зниження настрою з тривогою, фобіями, обсесіями, смутком, тугою, байдужістю більшу частину дня, зниження розумово-мовної та рухової активності, інтересу до навколишнього, ангедонію, втомлюваність, відчуття зневіри, ідеї малоцінності, суїцидальну активність, стійкі порушення сну, харчової поведінки, мають резистентні до лікування алгії, не підтверджені клінічними та інструментальними методами. В анамнезі відмічаються притаманні депресії спадковість, преморбід, важке соматичне або онкологічне захворювання, психічні травми, афективні розлади, АП та суїцидальні прояви. Клінічні дані підтверджуються експериментально-психологічними методиками (Люшера, Спілбергера, Гамільтона та ін.), даними біохімічних показників (характерні зміни дексаметазонової проби, рівня пролактину, перекисного окислення ліпідів, катехоламінергічної та серотонінергічної систем), електрофізіологічними дослідженнями (ЕЕГ, РЕГ), даними комп'ютерної (КТ) та магнітно-резонансної томографії (МРТ) [ 3, 4 ].Онкологічне захворювання є причиною широкого кола психічних розладів, які дебютують на ранніх етапах діагностики та формуються впродовж тривалого процесу лікування [10]. Серед них найчастіше зустрічаються пов'язані зі стресом розлади [7], в тому числі посттравматичні [9], дисоціативні розлади [5], нозогенні реакції [5,7]. Останнім часом зростає актуальність залучення психіатрів до проблеми ранньої діагностики та лікування депресивних розладів у пацієнтів на онкологічні захворювання [1,2].Поширеність депресії в популяції онкологічних пацієнтів складає від 33 до 42 % [1, 2].
