Введение. Дакриоцистоцеле (гидропс, водянка слезного мешка) – редкий вариант врожденной патологии, вызванной сочетанной обструкцией проксимального и дистального отделов слезоотводящих путей с последующим прогрессирующим растяжением слезного мешка. Вследствие скопления обильного содержимого в слезном мешке и носослезном протоке возможно растяжение мембраны, закрывающей его выход под нижнюю носовую раковину, и проминенция такой мембраны в нижний носовой ход в виде так называемой назолакримальной кисты. Цель. С учетом угрозы инфицирования содержимого дакриоцистоцеле, а также риска его осложнения респираторным дистресс-синдромом целью нашего исследования явился поиск оптимальных методов диагностики и возможных вариантов лечения данной патологии. Материалы и методы. Изучены результаты обследования и лечения 22 детей с дакриоцистоцеле. Во всех случаях патология выявлялась в первые сутки жизни. 3 (13,64%) из них имели жалобы на затруднение носового дыхания. Им же проводили МС КТ слезоотводящих путей с контрастированием в условиях севоранового наркоза. Все дети были обследованы отоларингологом и в ряде случав оперированы в условиях отоларингологического отделения. Результаты. В 27,27% дакриоцистоцеле опорожнялось при нажатии на область слезного мешка и далее не рецидивировало. У 72,73% детей потребовалось расширение слезных канальцев с эвакуацией содержимого слезного мешка. У 56,25% из них клиническая картина дакриоцистоцеле появилась вновь, что потребовало повторных вмешательств той же направленности. Вместе с тем у 3 детей (13,64% от числа всех обследованных) на протяжении первого месяца жизни наблюдались регулярные рецидивы дакриоцистоцеле, потребовавшие различных вмешательств и необходимости дополнительного лучевого обсле- дования. В результате у таких детей была диагностирована назолакримальная киста. В условиях оториноларингологического отделения всем таким пациентам выполнено эндоскопическое удаление назолакримальных кист, реконструкция слезоотводящих путей с их временной интубацией силиконовой нитью. Обсуждение. Лечение дакриоцистоцеле является неотложным. По результатам выполненного исследования предложен алгоритм ведения детей с данной врожденной патологией. Заключение. Взаимодействие офтальмолога и отоларинголога на всех этапах лечебно-диагностического процесса, в том числе оперативного лечения, позволяет локализовать препятствие на пути оттока слезы, целенаправленно осуществить реконструкцию слезоотводящих путей, избежать осложнений заболевания, а также излишних хирургических и терапевтических манипуляций.
Introduction. Dacryocystocele (hydrops, hydrops of the lacrimal sac) is a rare variant of congenital pathology caused by combined obstruction of the proximal and distal lacrimal ducts, followed by progressive stretching of the lacrimal sac. Due to the accumulation of abundant contents in the lacrimal sac and nasolacrimal duct, it is possible that the membrane covering its exit under the inferior nasal turbinate can stretch and prominence of such a membrane into the lower nasal passage in the form of the so-called nasolacrymal cyst. The purpose. Considering the threat of infection of the contents of dacryocystocele, as well as the risk of its complications with respiratory distress syndrome, the purpose of our study was to search for optimal diagnostic methods and possible treatment options for this pathology. Materials and methods. The results of examination and treatment of 22 children with dacryocystocele were studied. In all cases, the pathology was detected on the first day of life. 3 (13.64%) from the bottom had complaints of difficulty in nasal breathing. They also performed multislice computed tomography of the lacrimal ducts with contrast under sevoran anesthesia. All children were examined by an otolaryngologist and, in some cases, operated on in the otolaryngology department. Results. In 27.27%, the dacryocystocele emptied when pressing on the area of the lacrimal sac and did not recur. In 72.73% of children, dilatation of the lacrimal canaliculi with evacuation of the contents of the lacrimal sac was required. In 56.25% of them, the clinical picture of dacryocystocele reappeared, which required repeated interventions in the same direction. However, in 3 children (13.64% of all examined) during the first month of life, regular relapses of dacryocystocele were observed, which required various interventions and the need for additional radiation examination. As a result, these children were diagnosed with a nasolacrimal cyst. In the conditions of the otorhinolaryngology department, all such patients underwent endoscopic removal of nasolacrimal cysts, reconstruction of the lacrimal ducts with their temporary intubation with silicone thread. Discussion. Treatment of dacryocystocele is urgent. Based on the results of the study, an algorithm for the management of children with this congenital pathology was proposed. Conclusion. Interaction between an ophthalmologist and an otolaryngologist at all stages of the diagnostic and treatment process, incl. surgical treatment, allows you to localize the obstacle to the outflow of tears, purposefully carry out reconstruction of the lacrimal ducts, avoid complications of the disease, as well as unnecessary surgical and therapeutic manipulations.
