Serum β₂-microglobulin concentrations were determined in samples of 65 patients with benign or malignant monoclonal gammopathy. In the group of patients suffering from multiple myeloma or Waldenström’s macroglobulinemia the mean β₂-microglobulin level was significantly higher than in the group with benign monoclonal gammopathy. Values above 3 mg/l were highly indicative of malignant disease and observed in 50% of the myeloma patients. Serum creatinine levels were significantly correlated to β₂-microglobulin levels. However, mean creatinine concentrations did not significantly differ between the two groups of patients. Plasma cells and lymphoplasmocellular elements containing cytoplasmic immunoglobulin were counted in bone marrow samples of all patients. The counts, expressed in percent of nucleated bone marrow cells, allowed a good discrimination between the benign and the malignant group of patients. Bone marrow from patients with multiple myeloma or macroglobulinemia contained more, from patients with benign monoclonal gammopathy less than 17% plasma cells. No significant correlation was noticed between the extent of this plasmocytic bone marrow infiltration and serum β₂-microglobulin or creatinine levels.

We have investigated the regulation of β₂-microglobulin (β₂-M) synthesis by monocytes. Recent interest in β₂-M has developed since the discovery that this protein forms amyloid fibrils in patients undergoing long-term, chronic hemodialysis. The β₂-M amyloid-osis is linked to the greatly elevated levels of monomeric β₂-M in their circulation. Since factors that govern β₂-M release from plasma membranes are not known, we endeavored to evaluate β₂-M release during monocyte activation. Utilizing a human monocyte-like cell line, U937, we studied the effect of bacterial toxin stimulation on levels of membrane, cell surface, and supernatant β₂-M. We now present a novel method to purify β₂-M, a solid-phase radioimmunoassay to measure soluble β₂-M, and an ELISA to measure membrane β₂-M. Using these methods we found that the levels of β₂-M in the cell membrane or on the cell surface did not change during monocyte activation. However, activation did induce a significant increase in the concentration of β₂-M in monocyte supernatants, indicating that β₂-M synthesis by monocytes is increased during monocyte activation. These results suggest that monocyte activation by hemodialysis membranes may be a contributing factor to the observed increase in circulating β₂-M levels.

SUMMARY.A simple immunoturbidimetric method for the measurement of beta-2 microglobulin in serum on a centrifugal analyser is described. The method detects beta-2 microglobulin in the range 0·4-16 mglL with an estimated within-batch and between-batch coefficient of variation of 5-8·5% and 9%, respectively. The method correlated well with two ELISA methods.Beta-2 microglobulin (B2M) is a protein of relative molecular mass II 800 found in low concentrations in serum, urine and other body fluids." 2 It has been shown to constitute the common chain of class I MHC molecules found on the surface of all nucleated cellsr': 4 Free B2M in the serum is filtered at the glomerulus and almost completely reabsorbed and catabolised by the proximal renal tubule cells." 6 The serum level is inversely related to the glomerular filtration rate 7 and has been shown to be a reliable early indicator of renal glomerular disease. A recent trial showed that the serum B2M was the single most important prognostic factor in the disease. 14
