“…Prior to trophoblast invasion, the walls of spiral arteries undergo a variety of reorganization processes including widening of the vessel lumen (Brettner 1964), swelling of smooth muscle cells (Craven et al 1998) and vacuolation of endothelial cells (Boyd and Hamilton 1967). During the invasion period, a subpopulation of EVT, endovascular trophoblasts , invades from the uterine interstitium into maternal spiral arteries and subsequently lines and remodels them.…”

“…It is known that as soon as implantation begins decidualisation occurs within the stromal cells and in the ground substance of the endometrium. It is thought that prior to endovascular trophoblast migration, there is true decidualisation of the spiral arteries of the endometrium (Brettner 1964) and other structural changes occur in the myometrial spiral arteries (Brosens 1975). The signal intensity changes and the loss of integrity of the normal zonal uterine anatomy demonstrated by MRI may be due to these local changes in blood flow underlying the implantation site.…”

“…Theories started with a joint blood system of the mother and fetus and only in the 18th century did the separation of both blood systems become apparent (Hunter, 1980). In the mid 20th century, further evidence was provided suggesting that the uterine spiral arteries undergo morphological alterations even prior to the presence of any extravillous trophoblast cell near such vessels (Boyd & Hamilton 1956, 1967; Brettner 1964; Harris & Ramsey, 1966). These trophoblast‐independent alterations comprised endothelial vacuolation and swelling of smooth muscle cells.…”

“…All these changes lead to a vasodilatation of a certain degree, which is considered to be further increased by the activation of the local decidual artery renin‐angiotensin systems of the mother (Morgan et al. 1998) linked to early endometrial decidualization (Brettner 1964).…”
