Zomepirac Acyl Glucuronide Is Responsible for Zomepirac-Induced Acute Kidney Injury in Mice

Abstract: Glucuronidation, an important phase II metabolic route, is generally considered to be a detoxification pathway. However, acyl glucuronides (AGs) have been implicated in the toxicity of carboxylic acid drugs due to their electrophilic reactivity. Zomepirac (ZP) was withdrawn from the market because of adverse effects such as renal toxicity. Although ZP is mainly metabolized to acyl glucuronide (ZP-AG) by UDP-glucuronosyltransferase, the role of ZP-AG in renal toxicity is unknown. In this study, we established a… Show more

“…Although withdrawn from the market due to its tendency to cause serious anaphylaxis in an unpredictable subset of the population, zomepirac is still being actively researched and recent publications have focused on kidney toxicity using mice pretreated with esterase and glutathione synthesis inhibitors (Iwamura et al, 2016). In these experiments, zomeripac acyl glucuronide concentrations in the kidney correlated with biomarkers of kidney injury.…”

“…SOD activity was determined by the inhibition of the formation of the watersoluble formazan, which was evaluated by measuring the absorbance at 450 nm. The levels of lipid peroxidation in muscles were analyzed by evaluating malondialdehyde (MDA) concentrations using a TBARS assay kit (Oxford Biomedical Research) according to previously described methods (Iwamura et al, 2016).…”

“…Recently, AG-mediated cytotoxic mechanisms involving oxidative stress have been reported. 36,37 In addition, AGs play roles in drug–drug interactions via membrane transporters 17,38,39 as well as through CYP2C8 inhibition. 4042 Various factors that affect the safety of AGs, including the rates of formation, elimination, and aglucuronidation; their intrinsic reactivity; and the potential proteins being targeted, 43 as well as the molecular mechanisms underlying AG-induced toxicities, 44 all remain to be elucidated fully.…”
