The number of older adults is projected to grow significantly by 2050. Predictions also suggest that living longer but not healthier lives will increase the demand for family caregivers as well. Regrettably, caregivers in comparison with noncaregivers will likely continue to experience higher levels of stress, poorer health, and increased propensity for using unhealthy behaviors to address their stress. This study used mediation analysis to examine the relationship among 3 leisure variables, perceived stress, and self-reported general health (SRGH) of family caregivers (n = 240) who completed an online survey. Results indicated that individually, each of the 3 leisure variables was positively associated with higher SRGH. Results also showed that perceived stress significantly predicted SRGH. Of greater significance, however, was the observation that perceived stress was significantly predicted by both satisfaction with time for leisure and satisfaction with the quality of the leisure experience but not by leisure participation. Thus, the variables that addressed caregivers’ satisfaction with both the time for leisure and with the quality of the leisure experience were more effective than mere leisure participation to reduce perceived stress and ultimately provide better self-reports of health. These results are consistent with the literature about the ability of leisure to cope with stress. More importantly, they provide strong support for the design and implementation of interventions to help caregivers develop meaningful leisure-based activities to reduce perceived stress.

Background: Assessment of personality change in Alzheimer's disease and related dementias (ADRD) is clinically meaningful but complicated by patient (i.e., reduced insight) and informant (i.e., caregiver burden) factors that confound accurate reporting of personality traits. This study assessed the impact of caregiver burden on informant report of Big Five personality traits (Extraversion, Agreeableness, Conscientiousness, Neuroticism, and Openness) and investigated regional cortical volumes associated with larger discrepancies in patient and informant report of Big Five personality traits. Methods: Sixty-four ADRD participants with heterogeneous neurodegenerative clinical phenotypes and their informants completed the Big Five Inventory (BFI). Caregiver burden was measured using the Zarit Burden Interview (ZBI). Discrepancy scores were computed as the absolute value of the difference between patient and informant ratings for all BFI trait scores and summed to create a global score. Regional grey matter volumes from T1-weighted 3T MRI were normalized to intracranial volume and related to global Big Five discrepancy scores using linear regression. Results: Higher levels of caregiver burden were associated with higher informant ratings of patient Neuroticism (β =0.27, p =.016) and lower informant ratings of patient Agreeableness (β =-0.32, p =.002), Conscientiousness (β =-0.3, p =.002), and Openness (β =-0.34, p =.003) independent of disease severity. Patients with greater Big Five discrepancy scores showed smaller cortical volumes in right medial PFC (β = -0.00015, p = .002), right superior temporal gyrus (β = -0.00028, p = .025), and left inferior frontal gyrus (β = -0.00006 p = .013). Conclusions: Informant ratings of personality traits in ADRD can be confounded by caregiver burden, highlighting the need for more objective measures of personality and behavior in dementia samples. Discrepancies between informant and patient ratings of personality may additionally reflect loss of insight secondary to cortical atrophy in frontal and temporal structures.

Objetivo: Este estudo transversal objetivou avaliar a ocorrência de cárie dentária em 167 crianças de 6 a 12 anos de idade com paralisia cerebral e sua relação com a sobrecarga dos cuidadores. Métodos: Os dados foram coletados através de questionários e exames clínicos. A sobrecarga dos cuidadores foi avaliadautilizando-se a Escala de Avaliação da Sobrecarga do Cuidador - Burden Interview. Para a análise estatística foram utilizados os testes Qui-quadrado de Pearson, Exato de Fisher, F(ANOVA) e Student t-test. Resultados:A maioria das crianças apresentou paralisia cerebral do tipo espástica (70,7%). As prevalências de cárie foram 61,1% e 26,3% para as dentições decídua e permanente, respectivamente. A maioria dos cuidadores apresentou algum nível de sobrecarga (80,5%), sendo que o maior percentual apresentou sobrecarga leve à moderada (55,0%) e 25,5% apresentaram sobrecarga moderada a severa. Verificou-se que as médias do índice CPO-D e de dentes permanentes cariados foi maior entre os que tinham hemiparesia e menor entreos que tinham quadriparesia, com diferença significativa entre essas duas categorias (p=0,004). A ocorrência de cárie dentária não foi associada à sobrecarga dos cuidadores (p=0,881 para a dentição decídua e p=0,144para a dentição permanente). A sobrecarga do cuidador foi associada à escolaridade (p=0,044) e à região de procedência (p=0,001). Conclusão: Nesta população, a cárie está mais relacionada aos problemas inerentesaos comprometimentos da paralisia cerebral, do que às dificuldades da rotina do cuidador. Sugere-se que estratégias mais humanizadas de acolhimento aos cuidadores sejam traçadas, visando à abordagem integral dos pacientes com paralisia cerebral e suas famílias.Descritores: Paralisia cerebral.
