Yield, Techniques, and Complications of Transvitreal and Transscleral Biopsies in Small Uveal Melanoma

Abstract: Intraoperative adequacy check of biopsy samples can lead to a higher biopsy yield rate, and subsequent genomic analysis can provide patients with valuable prognostic information. OBJECTIVES To examine the yield rates for transscleral and transvitreal fine-needle aspiration biopsies of small uveal melanoma less than 3.6 mm in apical height and to discuss techniques that would maximize yield rates and minimize complications. DESIGN, SETTING, AND PARTICIPANTS A retrospective, consecutive observational case series… Show more

“…Briefly, visual acuity was 20/40 OU or better in 1276 of the patients (71.7%), and the mean (SD) visual acuity was 20/40 (20/50) OU (median, 20/30; range, 20/ 20 to counting fingers). The mean (SD) patient age at initial presentation was 58 (14) years (median, 59 years; range, 10-93 years). Most patients were white (n = 1752; 98.4%), and male and female participants were equally represented (n = 908 [51.0%] were female).…”

“…9,[15][16][17][18][19][20][21] The transvitreal pars plana technique may be performed using a microscope or under indirect ophthalmoscopy, using standard vitrectomy techniques or directly with a 23-gauge (G)/25G/27G needle. [15][16][17][18]20,21 Treatment of the tumor may be performed before or after FNAB. 9 Risks associated with the transvitreal biopsy include vitreous hemorrhage, cataract, rhegmatogenous retinal detachment, and hypotony.…”

[15][16][17][18]20,21 Transscleral biopsies may be performed under direct visualization, using magnifying loupes, or under the microscope. 17,18,20,21 Some surgeons may perform a scleral dissection at the time of the transscleral FNAB procedure.…”
