Xanthogranulomatous Pyelonephritis: Synchronous Upper and Lower Gastrointestinal Bleed

Abstract: Xanthogranulomatous pyelonephritis (XGP) is a rare chronic granulomatous destructive process of the renal parenchyma. It is caused by a chronic inflammatory process due to recurrent urinary tract infections and/or obstructing renal calculi. Rarely, it presents with advanced complications including abscesses and fistula formations. In this article, we report a unique presentation of XGP with simultaneous upper and lower gastrointestinal bleeding in the setting of XGP with reno-gastric and reno-colic fistulas.

“…The presence of a reno-alimentary fistula not only increases the risk of urinary tract infections but also promotes abscess formation due to the interaction between urine and digestive contents, creating an environment suitable for bacterial growth. Abscesses result in significant pain, swelling, and general illness [ 9 , 12 ]. Furthermore, severe cases of reno-alimentary fistulas can lead to life-threatening sepsis, characterized by systemic infection, causing widespread inflammation, organ dysfunction, and potential fatality if untreated [ 13 ].…”

“…Three different types of XGP are described as the majority are diffuse (90%), with segmental and focal together constituting around 13% of cases [3]. It is one of the challenging diseases to diagnose because the presentation is non-specific with variable clinical features, laboratory abnormalities, and radiological findings [4]. Infliximab is a genetically engineered monoclonal antibody targeted to inhibit the proinflammatory cytokine, tumor necrosis factor-alpha (TNF-α) [5].…”

“…XGP has two morphologic subtypes, namely, the focal and diffuse forms, which may extend outside the kidney and can be complicated with fistula formation connecting the urinary tract with the proximate organs ( 1 ). The occurrence of fistulas connecting the kidney to the upper and lower gastrointestinal tract, bronchus, diaphragm, psoas muscle, and skin has been reported ( 2 3 ).…”
