Abstract: Dermatofibrosarcoma protuberans, atypical fibroxanthoma, and cutaneous leiomyosarcoma are soft tissue sarcomas that have a high rate of recurrence when treated with standard surgical techniques and may occasionally metastasize. When removing these tumors it is very important to take adequate margins. Mohs micrographic surgery is proving to be very useful in treating these difficult neoplasms.

“…Because they are dermal-based tumors, they may appear fixed to the epidermis and ulceration may be present. [20] The lower extremities and head and neck locations are the most common sites of involvement as identified in our series and in the literature. [3, 14, 21] Pre-operative staging with cross-sectional imaging (CT thorax and/or PET imaging) appeared to add minimal clinical impact in our experience as more than half of the patients in this series underwent imaging, all of which were negative.…”

“…[14] Others have suggested using a 3-5 cm margin of excision because of the potential risk of local recurrence and distant metastasis. [20] Narrow margin excision[13] and even Mohs micrographic surgery have also been used. [10, 11, 28] While several small case series using Mohs resection have been described, recurrences have been reported with long-term follow up.…”

“…In general, dermal tumors appear adhered to the epidermis with frequent ulceration of skin with or without crusts, while subcutaneous tumors are mobile. [15] Our patient was a 77-year-old male who presented with a sacral mass of 7 cm × 5 cm × 5 cm, with brownish discoloration and ulceration of the skin. As he was a farmer by occupation, there is possibility of trauma, which might have gone unnoticed in his case.…”

“…These tumours are locally aggressive and highly recurrent, but metastases or tumour-related deaths are extremely rare (Enzinger and Weiss, 1995;Fish, 1996;Mentzel et al, 1998). DP presents speci®c cytogenetic features such as reciprocal translocations t(17;22)(q22;q13.1) or more often, supernumerary ring chromosomes derived from t(17;22) (Pedeutour et al, 1995(Pedeutour et al, , 1996.…”
