With more than 20 million patients annually, inguinal hernia repair is one of the most often performed surgical procedures worldwide. The lifetime risk to develop an inguinal hernia is 27-43% for men and 3-6% for women. In spite of all advances, 11% of all patients suffer from a recurrence and 10-12% from chronic pain following primary inguinal hernia repair. By developing evidence-based guidelines and recommendations, the international hernia societies aim to improve the outcome of inguinal hernia repair due to standardization of care. From a total of more than 100 different repair techniques for inguinal and femoral hernias, classified as tissue repair, open mesh repair, and laparo-endoscopic mesh repair, the new International Guidelines of the Hernia-Surge Group only recommend the totally extraperitoneal patch plasty (TEP), transabdominal preperitoneal patch plasty (TAPP), and Lichtenstein techniques. Since a generally accepted technique suitable for all inguinal hernias does not exist, surgeons should provide both an anterior open (Lichtenstein) and a posterior laparo-endoscopic (TEP or TAPP) approach option. The guidelines strongly recommend that surgeons tailor the treatment of inguinal hernias based on expertise, local/national resources, and patient- and hernia-related factors. A tailored approach in inguinal hernia repair should pay heed to the patient- and hernia-related factors, unilateral hernia in men and women, bilateral hernia, recurrent hernia, scrotal hernia, previous pelvic and lower abdominal surgery, severe cardiac or pulmonary comorbidities, and incarcerated hernia.

Strangulated lumbar hernia is a very rare condition, with no more than 30 cases reported in the literature so far. Therefore, there is no specific management guideline and the diagnosis remains difficult. By reporting the case of a Senegalese male patient who had a preoperative diagnosis of strangulated lumbar hernia, we aim to discuss the diagnosis and therapeutic modalities of this rare entity, which is often misdiagnosed.

S oc ied a d Hi s p a n oa me r ic a n a d e H ernia ResumenEl tratamiento de las hernias ventrales complejas contaminadas o muy contaminadas es un reto aún sin una estrategia técnico-quirúrgica defi nida en la bibliografía médica publicada. Presentamos el caso de un hombre con una hernia ventral compleja muy contaminada de la prótesis sintética por Staphylococcus aureus resistente a meticilina, en el que se optó por una cirugía en dos etapas con una semana intercalada de tratamiento antibiótico. En el posoperatorio desarrolló un seroma tratado de forma conservadora, y a los 6 meses de seguimiento no se evidenció recidiva, abombamiento de la pared abdominal ni rechazo de la prótesis. El resultado favorable obtenido con esta estrategia elegida (técnica en dos etapas) induce a conjeturar si el manejo terapéutico seguido en este paciente podría ser efectivo para evitar demasiado tiempo de convalecencia en hernias incisionales complejas contaminadas. AbstractThe treatment of contaminated complex ventral hernias in a heavily contaminated setting is highly challenging and still without a clear strategy in the literature. We present a case where we treat a complex ventral hernia with synthetic mesh with major contamination by methicillin-resistant Staphylococcus aureus in which we opted for a dual-stage approach separated by a period of a week with antibiotic treatment. The patient initially developed a seroma treated conservatively and six months later shows no sign of recurrence or bulging. The favorable outcome of this approach may avoid the commonly practiced convalescence period of months in cases of major local contamination.
