El bileği için enjeksiyon tanıdan tedaviye kadar birçok sebep için yapılabilir. Özellikle osteoartrit ve enflamatuar artrit hastalarında ağrı tedavisi ve fonksiyonel düzelme için kortikosteroid ve analjezik tedavisi, kartilaj problemleri için hyalunorik asit tedavisi gibi tedavi edici yöntemler ÖZET Amaç: Bu çalışmamızda, ultrasonografi eşliğinde ve palpasyon ile yapılan enjeksiyonları, MR artrografi ile değerlendirerek karşılaştırmayı ve her iki teknik arasında işlemin başarısı ve ekstravazasyon oranları açısından fark olup olmadığını ortaya koymayı amaçladık. Gereç ve Yöntem: Bu prospektif çalışmamızda klinik endikasyon dahilinde el bileği enjeksiyonu yapılan hastaların çalışmaya dahil edilmesi planlandı. Çalışma sürecinde 40 hastaya, iki gruba ayrılarak, yarısına ultrasonografi eşliğinde diğer yarısına palpasyon yöntemi ile enjeksiyon yapıldı. Enjeksiyon sonrası hastalara MR artrografi çekilerek, kontrast maddenin eklem içerisine ulaşıp ulaşmadığı ve ekleme komşu yumuşak dokular içerisine ekstravazasyonu değerlendirildi. Bulgular:Ultrasonografi eşliğinde yapılan enjeksiyonların hepsinde eklemde kontrast madde varlığı mevcuttu (%100). Palpasyonla enjeksiyon yapılan hastaların 17 tanesinde eklemde kontrast madde izlendi (%85). Ultrasonografi eşliğinde enjeksiyon yapılan üç (%15) hastada iğne trasesinde kontrast madde ekstravazasyonu mevcut iken bu sayı palpasyon metodu ile yapılan enjeksiyonlardada beşti (%25). Sonuç: El bileğine palpasyon ve ultrasonografi eşliğinde yapılan enjeksiyonlarda işlemin başarı ve ekstravazasyon oranları arasında anlamlı farklılık mevcut değildi. Palpasyon zaman ve maliyet açısından etkin bir yöntemdir. Fakat bu yöntem eklemin palpasyonunu etkileyen patolojilerin varlığında yetersiz kalmakta ve böyle durumlarda ultrasonografi kullanımı enjeksiyon başarısını artırmaktadır. ABSTRACTObjective: In this study, we aimed to compare the ultrasound guided injection and injection with palpation using MR arthrography regarding the differences in success rate and the extravasation rate of the two techniques. Materials and Methods: In this prospective study, we planned to include patients to whom an injection to the wrist was performed due to an indication. First, forty patients were divided into two groups and, ultrasound guided injection was performed to one group whereas injection was performed with palpation to the other. After the procedures, MR arthrography was performed to assess the reach of the contrast in to the joint space as well as extravasation into the adjacent soft tissues. Results: There was contrast in the joint space in all injections guided by ultrasound (100%). Seventeen out of 20 injections which performed with palpation, there was contrast within the joint space (85%). Extravasations were seen in 3 patients (15%) with ultrasound guided injections group and 5 patients (25%) with palpation group. Triangular fibrocartilage complex is a major stabilizer of the distal radioulnar joint (DRUJ).However, triangular fibrocartilage complex (TFCC) tear is difficult to be diagnosed on MRI for its intrinsic small and thin structure with complex anatomy. The purpose of this article is to review the anatomy of TFCC, state of art MRI imaging technique, normal appearance and features of tear on MRI according to the Palmar's classification. Atypical tear and limitations of MRI in diagnosis of TFCC tear are also discussed.

Abstract: Wrist MR arthrography with axial traction significantly improved the visibility of articular cartilage and the detection and visibility of tears of the TFCC and intrinsic ligaments. The results favor more widespread use of traction during MR arthrography of the wrist.
