Woven coronary arteries: A detailed review

Abstract: A woven coronary artery is a rare congenital vascular variation that involves the separation of the artery into several thin channels, which later merge distally to form a single lumen. This mesh of intertwining small vessels may occur anywhere along the vessel and may be present in multiple coronaries. Although generally a benign incidental finding, there have been reported cases of associated ischemia, thrombus and atherosclerotic plaque formation and complications with cardiac intervention procedures. In th… Show more

“…However, it has been reported to be associated with thrombus and myocardial infarction [5]. PCI of a local or short lesion with woven change has been reported in few publications [6]. Most cases of woven coronary artery were noticed accidentally during coronary angiography, and now the diagnostic criteria is based on angiographic images.…”

“…In the literature, there have been 2 main proposed theories regarding pathology. Some believe that this disorder is congenital and more specifically sporadic as no reported cases are identifying a genetic predisposition or hereditary cause [ 1 ] [ 6 ]. Others believe that it is a consequence of spontaneous cardiac dissection or recanalized thrombus [ 1 , 7 ].…”

“…Others believe that it is a consequence of spontaneous cardiac dissection or recanalized thrombus [ 1 , 7 ]. The causes of WCAA can be vasculitic disorders such as spontaneous coronary artery dissection, recanalized thrombus, or Kawasaki disease, and it may be associated with long-term chronic conditions such as rheumatoid arthritis [ 1 ] [ 2 ] [ 3 ] [ 6 , 8 ]. It is important to understand that WCAA can be due to spontaneous cardiac dissection or recanalized thrombus but can also mimic those pathological states.…”

“…Therefore, a reasonable etiology was in situ thrombosis secondary to coronary dissection, plaque rupture, or proximal coronary ectasia. HLS should be differentiated from woven coronary artery (WCA), 21 another very rare condition that is recognized as a congenital anomaly without prognostic significance. In most cases, the involved WCA has normal coronary blood flow and the patient is asymptomatic.…”
