Working intentions of medical students in response to healthcare workplace violence and descending resources reform in China

Background In order to curb healthcare workplace violence (WPV) and better allocate healthcare resources, China launched the descending resources reform in 2013 and tightened the anti-violence legal environment simultaneously. Medical students are expected to reconsider their working intentions of entering the medical market (inter-market effect) and choosing high- or low-level hospitals (intra-market effect) in response to the evolving WPV. The goal of this study was to explore the link between the perceived WPV incidence and medical students’ willingness to practice medicine in the context of China’s descending resources reform. Method Medical students were selected with cluster sampling from 8 medical colleges in Zhejiang Province, China, and 1497 valid questionnaires were collected by using a five-point unbalanced scale, to perform cross-sectional empirical research using the ordered logit model (OLM). Results The perceived WPV incidence negatively correlate with the willingness of medical students to practice medicine but positively correlate with their willingness to practice in low-level hospitals, indicating the existence of inter- and intra-market effects. The anti-violence legal environment has no direct link with working intention but contributes to the perceived decline in the incidence of violence. Descending resources reform has simultaneous opposite effects on medical students, with the coexistence of prudent motives driven by reform costs and optimistic expectations of sharing external benefits. Conclusions Safety needs and risk aversion motive play an important role in medical students’ career choice when facing severe WPV. Tightening of the anti-violence legal environment and the descending resources reform could drive medical students to low-level hospitals.

BackgroundStudies have reported that empathy had a positive effect on professional identity (PI) in nursing students. However, little was known about the mechanism underlying this relationship between empathy and professional identity in nursing students.ObjectiveThe purpose of this study was to analyze in depth the mediating effect of workplace violence (WVP) between empathy and professional identity in nursing students.MethodsA total of 405 nursing students participated and were investigated using the Chinese version of the Jefferson Scale of Empathy-Health Professional (JSE-HP), the scale of professional identity about nursing students, and the workplace violence Incident Survey in this study. Hierarchical regression was used to analyze the mediating effect of workplace violence on the relationship between empathy and professional identity among nursing students.ResultsThe score of nursing students' professional identity was 103.69 ± 17.79. Workplace violence had a significant negative correlation with empathy (r = −0.449, P < 0.001) and professional identity (r = −0.330, P < 0.001). Workplace violence accounted for 14.59% of the total mediating effect on the relationship between empathy and professional identity for nursing students.ConclusionsIn this study, the level of professional identity of nursing students was low. Workplace violence had a partially significantly mediating effect on the relationship between empathy and professional identity. Maybe, it was suggested that nursing students' professional identity might be improved and driven by a decrease in workplace violence. Targeted interventions at reducing nursing students' workplace violence should be developed and implemented. In addition, nursing managers and educators should be aware of the importance of empathy and improve professional identity in nursing students.

Abstract Nursing students have the potential for violence in places where they carry out their clinical practice, both in hospitals and in communities. The competence of nursing students needs to be reviewed to see their readiness before, time, and after events. This study aims to measure the competence of students of the profession nurse (Ners) to face violence at hospitals. The study is a descriptive quantitative study, with a sample number of 55 students of the profession nurse. The analysis using the management questionnaire of genealogical violence scale (MWVCS) indicates that students of the profession have a high level of competence and a high level of competence. The factors that indicate very high competence are 'after the violence in the hospital' and 'the interactions between nurses and patients. While the factors that indicate high competence are 'response to violence', 'cognitive to violence', 'exploiting facilities of protection', 'improving knowledge', and 'risk assessment'. Nursing education needs to prepare the student to anticipate a violent episode and teach them how to anticipate the violence. Keywords: Competency factors, nursing students, workplace violence Abstrak Mahasiswa keperawatan memiliki potensi mengalami kekerasan di tempat dimana mereka melaksanakan praktik klinisnya, baik di rumah sakit maupun di komunitas. Kompetensi mahasiswa perawat perlu dikaji untuk melihat kesiapan mereka sebelum, saat dan sesudah kejadian. Tujuan dari penelitian ini adalah untuk mengukur tingkat kompetensi mahasiswa profesi Ners dalam menghadapi tindakan kekerasan di rumah sakit. Penelitian ini merupakan penelitian deskriptif kuantitatif, dengan jumlah sampel 55 orang mahasiswa profesi Ners. Hasil analisa dengan menggunakan kuesioner Management of Workplace Violence Competence Scale (MWVCS), menunjukkan bahwa mahasiswa profesi Ners memiliki tingkat kompetensi tinggi dan sangat tinggi. Faktor yang menunjukan kompetensi sangat tinggi adalah faktor ‘setelah terjadi kekerasan di rumah sakit’ dan ‘interakasi antara perawat dan pasien’. Sedangkan faktor yang menunjukkan kompetensi tinggi adalah faktor ‘respon terhadap kekerasan’, ‘kognitif terhadap kekerasan’, ‘pemanfaatan fasilitas perlindungan’, ‘pembaharuan pengetahuan’, dan ‘pengkajian resiko’. Pendidikan keperawatan perlu mempersiapkan mahasiswanya dalam mengantisipasi kejadian kekerasan dan mengajarkan mereka cara mengantisipasi kejadian kekerasan tersebut.
