Health care systems need to be resilient to deal with disasters like the global spread of the Severe Acute Respiratory Syndrome Coronavirus (SARS-CoV-2) on top of serving the changing needs of a multi-morbid, ageing and often dispersed population. This paper identifies, discusses and augments critical dimensions of resilience retrieved from the academic literature. It pulls together an integrated concept of resilience characterised by organisational capabilities. Our concept does not focus on the micro-level like most resilience literature in health care but addresses the system level with many stakeholders involved. Distinguishing exogenous shocks to the health care system into adverse events and planned innovations provides the basis for our conclusions and insights. It becomes apparent only when dealing with planned interventions that transformative capabilities are indispensable to cope with sudden increases in health care pressures. Due to the current focus on absorptive and adaptive resilience, organisations over-rely on management capabilities that cannot generate a lasting increase in functionality. Therefore, reducing the resilience discussion to bouncing back from adverse events could deceive organisations into cultivating a suboptimal mix of organisational capabilities lacking transformative capabilities, which pave the way for a structural change that aims at a sustainably higher functionality.

Introduction: Growing numbers of older patients occupy hospital beds despite being 'medically fit' for discharge. These Delayed Transfers of Care amplify inefficiencies in care and can cause harm. Delayed transfer because of family or patient choice is common; yet, research on patient and family perspectives is scarce. To identify barriers to, and facilitators of, shorter hospital stays, we sought to understand older people's and caregivers' thoughts and feelings about the benefits and harms of being in hospital and the decisions made at discharge.Methods: A multimethod qualitative study was carried out. Content analysis was carried out of older people's experiences of health or care services submitted to the Care Opinion online website, followed by telephone and video interviews with older people and family members of older people experiencing a hospital stay in the previous 12 months.Results: Online accounts provide insight into how care was organized for older people in the hospital, including deficiencies in care organization, the discharge process and communication, as well as how care was experienced by older people and family members. Interview-generated themes included shared meanings of hospitalization and discharge experiences and the context of discharge decisions including failure in communication systems, unwarranted variation and lack of confidence in care and lack of preparation for ongoing care. Eye-SCOR: A Supply Chain Operations Reference-Based Framework for Smart Eye Status Monitoring Using System Dynamics Modeling

This work is a unique integration of three different areas, including smart eye status monitoring, supply chain operations reference (SCOR), and system dynamics, to explore the dynamics of the supply chain network of smart eye/vision monitoring systems. Chronic eye diseases such as glaucoma affect millions of individuals worldwide and, if left untreated, can lead to irreversible vision loss. Nearly half of the affected population is unaware of the condition and can be informed with frequent, accessible eye/vision tests. Tonometry is the conventional clinical method used in healthcare settings to determine the intraocular pressure (IOP) level for evaluating the risk of glaucoma. There are currently very few (under development) non-contact and non-invasive methods using smartphones to determine the risk of IOP and/or the existence of other eye-related diseases conveniently at home. With the overall goal of improving health, well-being, and sustainability, this paper proposes Eye-SCOR: a supply chain operations reference (SCOR)-based framework to evaluate the effectiveness of smartphone-based eye status monitoring apps. The proposed framework is designed using system dynamics modeling as a subset of a new causal model. The model includes interaction/activities between the main players and enablers in the supply chain network, namely suppliers/service providers, smartphone app/device factors, customers, and healthcare professionals, as well as cash and information flow. The model has been tested under various scenarios and settings. Simulation results reveal the dynamics of the model and show that improving the eye status monitoring device/app factors directly increases the efficiency/Eye-SCOR level. The proposed framework serves as an important step towards understanding and improving the overall performance of the supply chain network of smart eye/vision monitoring systems.
