Intramedullary fixation is used increasingly to treat clavicular fractures. Anatomical variations in the clavicle of relevance to this procedure are analyzed. The length, diameters and curvature of the clavicle were measured in 196 specimens from the dissecting room. The calcium bone density was analyzed in 300 cross-sectional samples of 100 specimens. The thickness of cortical and medullary bone of 70 slices was analyzed from freshly plastinated clavicles with implants in place. The female clavicle was shorter, less curved, and had a lower concentration of calcium than the male clavicle. Measurement of cortical thickness showed a mean value of 1.05 +/- 0.23 mm at the most sternal measuring point, 2.05 +/- 0.29 mm at the midpoint of the clavicle, and 0.95 +/- 0.35 mm at the acromial end. The thinnest regions were the medial ventral cortex and the dorsal acromial cortex. These measurements explain clinical observations on nail perforation. The diameter of the medullary canal measured 6.7 +/- 2.6 mm at its narrowest part, so that reaming (i.e., predrilling the medullary canal) prior to 3.5 mm titanium nail insertion is not necessary. The main difficulties encountered when placing a nail are secondary to the S-curvature of the clavicle. In 80% of fractures, the break is located at the narrowest diameter of the medullary canal. Thus, the clavicle displays definite gender- and side-specific anatomical features in terms of length, diameter, curvature, and calcium concentration. These should be considered when performing intramedullary fixation.

Die Klavikulaschaftfraktur stellt mit 3-5% aller Frakturen und etwa 45% aller Schulterverletzungen eine der häufigs-ten Skelettverletzungen des Menschen dar. Ihre Inzidenz beträgt etwa 64 Fälle auf 100.000 Personen/Jahr [13]. Sie entsteht durch den Sturz auf die Schulter, sehr häufig als Folge eines Fahrradsturzes. Oft kommt es durch den Zug des M. sternocleidomastoideus zu einer erheblichen Fehlstellung mit Verkippung des körper-nahen Fragments nach oben und zu einer Verkürzung.Die Diagnose ist klinisch und radiologisch leicht zu stellen. TherapieFrüher wie heute galt der Satz:"Die Klavikulafraktur heilt mit Arzt, ohne Arzt und trotz Arzt!" Konservative BehandlungBis vor kurzem wurde die Schlüssel-beinfraktur konservativ durch Anlage eines Rucksackverbands für 4 bis 6 Wochen behandelt. Sie heilt in der Regel mit deutlicher Kallusbildung sowie Verkür-zung und Verkippung aus. Eine Reposition oder Stabilisierung gelingt im Rucksackverband entgegen der landläufigen Meinung nicht. Petracic [11] erklärte bereits 1983, wenn man eine Reposition im Rucksackverband erreichen wollte, müsse er so fest angezogen sein, dass beide Arme nicht mehr durchblutet werden würden. Der Verband führt zu Venenstau und erinnert den Patienten lediglich an das Vorhandensein einer Fraktur und strahlt bereits nach kurzer Zeit unangenehme Gerüche aus, weshalb wir ihn auch heutewenn überhaupt -nur sehr kurz für wenige Tage zur Linderung der Schmerzen anlegen.In verschiedenen Studien wurden bei der konservativen Behandlung der Klavikulafraktur Pseudarthroseraten von 1-13% beschrieben [13]. Trotzdem wurden bis vor 15 Jahren gut 80% der Frakturen konservativ behandelt. Als Operationsindikationen galten nur die offenen Frakturen, die drohende Durchspießung, Nerven-Gefäß-Störungen, die "floating shoulder" mit Skapula-und begleitender Klavikulafraktur sowie die instabile Rippenserienfraktur mit begleitender Klavikulafraktur, um VerfahrenswahlBiomechanisch ist die intramedulläre Applikation als ideal anzusehen, weil das Problem der wechselnden Zuggurtungsseite des Schlüsselbeins durch die zentrale Lage des Kraftträgers umgangen wird [5,6,13]. Geeignet für die intramedullä-re Schienung [ESIN, TEN ("titan elastic nail"), Prevot-Nagelung] sind grundsätz-lich nur 2-und 3-Fragment-Frakturen. Bei mehrfragmentären Frakturen ist das Verfahren nicht sinnvoll, da die sich die Fraktur ähnlich wie eine Ziehharmonika über dem Nagel zusammenstauchen kann und keine genügende Abstützung findet. Bei 3-Fragment-Frakturen ist auf eine ausreichende Abstützung zu achten, da es sich bei der Versorgung mit dem (unverriegelten) Nagel um ein längeninstabiles Verfahren handelt. Vorgehen NachbehandlungDie Versorgung ist bei sicherer Nagelverklemmung übungsstabil. Gelingt die geschlossene Reposition, können die Patienten ggf. ambulant behandelt werden. Die elastische Markraumschienung mit flexiblen Titannägeln (ESIN) zur Behandlung von dislozierten Klavikulafrakturen im mittleren Abschnitt erweist sich als eine Erfolg versprechende minimalinvasive Alternative zur Plattenosteosynthese. In der vorliegenden Studie werden die operative Technik, die Ergebnisse und die Komplikationen bei 55 Anwendungen dargestellt. Über einen medialen Zugang wurden 37 Frakturen offen und 18 geschlossen reponiert. Die Metallentfernung erfolgte nach durchschnittlich 18 Wochen bei bislang 44 Patienten. Der postoperative Funktionsstatus bei 52 Patienten nach 13 ± 4 Monaten betrug im ASES-Score 28 ± 4 Punkte (von maximal 30) und im Constant-Score 81 ± 11 (maximal 100). Eine achsengerechte Frakturheilung wurde in 47 (90 %) Fällen erzielt, eine verzögerte Kallusbildung trat einmal auf. Laterale primäre Implantatperforationen bestanden bei 12 (22 %) Patienten. Weitere Nageldislokationen traten 4-mal (7 %) auf. Insgesamt wurden zwei operative Revisionen, einmal in Form einer Neuplatzierung des Titanimplantates bei Nagelfehllage und einmal als Systemwechsel mit einer Plattenosteosynthese nach Implantatmigration, durchgeführt. Das wenig invasive Vorgehen der elastischen Markraumschienung führt zu einer raschen Schmerzfreiheit, bietet öko-nomische Vorteile bei kalkulierbaren Risiken, sodass diese Methode inzwischen in unserer Klinik als Standardverfahren etabliert ist. AbstractElastic stable intramedullary nailing (ESIN) of displaced midclavicular fractures with titanium nails is a minimal invasive treatment as an alternative for plate fixation. Here we illustrate the surgical technique, the results and complications of 55 treated patients. The nail was advanced from medial to lateral with an open reduction in 37 and closed in 18 cases. The removal of the titanium implant in 44 patients was performed after an average of 18 weeks. The functional status of 52 patients after 13 month reached 28 ± 4 of maximum 30 points in the ASES score and 81 ± 11 (maximum 100 points) in the Constant Score. Fracture healing with good reduction was gained in 47 (90 %) patients, and only one case with delayed consolidation. Intra-or postoperative lateral perforations of the nail tip were recorded in 12 patients. An additional migration of the nail was detected 4 times with implant removal and plate fixation in one patient. Another surgical revision was necessary to correct a titanium nail, which missed the lateral medullar canal. This minimal invasive treatment with immediate pain release, cost-effective advantages and minor risks has justified the ESIN methode to a standard treatment in our hospital. Originalarbeit 303
