Widespread Pressure Pain Hypersensitivity in Musculoskeletal and Nerve Trunk Areas as a Sign of Altered Nociceptive Processing in Unilateral Plantar Heel Pain

Abstract: This study found widespread pressure hypersensitivity over both nerve trunks and musculoskeletal structures in individuals with unilateral chronic plantar heel pain, as a manifestation of a centrally altered central nociceptive pain processing.

“…In our study, due to ineffective results showed in a previous treatment, exercise have not been included. However, in other studies [15,36], therapeutic exercise showed improvements in pain in patients with jumper's knee. Besides, therapeutic exercise -such as concentric and eccentric exercises -have been clearly proved to have an evidence for treatment of tendinopathies [1].…”

“…The mean of 3 tests on the most painful quadricipital tendon point was calculated and used for the analysis. Thirty seconds, similarly to other studies [15,16], was used between each test. A 30 s resting period was allowed between tests for avoiding temporal summation [17].…”

“…Neutral ankle dorsiflexion and supine decubitus position were applied. The mean of three repeated measurements, at 30–60 s intervals, was used for the analysis data based on earlier studies [17,18,19,20,21,22,23].…”

“…PPTs of the foot bones and soft tissues have shown the presence of central sensitization in patients with other medical conditions such as migraine [13], low back pain [14], knee osteoarthritis [15], and irritable bowel syndrome [16]. Considering the bone and soft tissue structures of the foot, the lateral malleolus for assessing bone PPT [17] and the tarsal tunnel (including plantar flexor tendons, blood vessels, and tibial nerve trunk) for evaluation of soft tissue PPT [18,19,20,21,22,23] may be considered as one of the most useful sites to assess PPT by means of algometry.…”

“…Future studies should also emphasise the importance of returning exercise diaries to monitor adherence [13]. Recently, several psychological and nutritional PHP risk factors have been identified, therefore future studies could investigate psychological and nutritional interventions alongside physiotherapy in a biopsychosocial treatment model [102][103][104][105][106][107][108][109][110]. ⨁⨁⨁⨁ HIGH *The risk in the intervention group (and its 95% confidence interval) is based on the assumed risk in the comparison group and the relative effect of the intervention (and its 95% CI).…”
