The aim of this study was to use ultrasonography to determine the locations and distributions of the superior labial artery (SLA) and the inferior labial artery (ILA) relative to the vermilion border (VB). Sixty healthy Korean volunteers (35 males, 25 females; age, 21-36 years) were investigated using ultrasonography. The participants had not received any noninvasive treatment or surgical procedure in the facial regions during the previous 6 months. Based on the VB, the overall thicknesses of the upper and lower lips were 9.4 AE 0.4 mm (mean AE SD) and 10.9 AE 0.7 mm, respectively. In most cases, the labial arteries were located in the wet mucosal layer on both the upper (35-57%) and lower lips (28-55%), respectively. In the upper lip, the SLA was in the intramuscular layer in 20-45% of cases, making it the second most common type. At some of the measuring points, the SLA was observed more often in the intramuscular layer than in the wet mucosal layer. In the lower lip, the ILA was also located in the dry mucosa (5-27%). The dry-wet mucosal junction is unclear in the lip area, and the ILA was commonly observed at the dry-wet mucosal junction. The arterial depth was 5.3 AE 0.3 mm in the upper lip and 4.2 AE 0.4 mm in the lower lip. The SLA and ILA are evenly distributed over all parts of the oral mucosa. Achieving predictable results with oral lip enhancement procedures is now possible. These procedures are categorized as either lip-reshaping or lip-voluming techniques. By performing these techniques appropriately and simultaneously, the limitations of both are overcome and ideal lip aesthetics can be obtained in one operative setting. An algorithmic approach is presented to facilitate choice of techniques and when to combine them. This article also highlights the combination concept, discussing lip-reshaping procedures such as multiple mucosal advancements, modified paranasal lip lifts, and corner lip lifts performed in concert with voluming by means of fat transfer. However, the main focus of this article is on the fat transfer and simultaneous mucosal advancement (FATMA) procedure, one that has produced remarkable results in terms of permanency and aesthetic appeal. One-year to 7-year follow-up results are presented.

Study objective: To establish the safety and efficacy of small-gel particle hyaluronic acid (SGP-HA; Restylane®, Medicis Aesthetics Inc., Scottsdale, AZ) for lip augmentation. Study design: This was a Phase 3, prospective, open-label, evaluator-blinded, single-center pilot study of SGP-HA use in lip augmentation. The primary efficacy objectives were to investigate the efficacy of SGP-HA in lip augmentation and to assess subject satisfaction with the procedure 12 weeks after treatment. Secondary efficacy objectives were to validate 3D imaging to measure lip augmentation, identify subject satisfaction at all points in time, and identify the duration of lip augmentation and palpability of SGP-HA in the lips. The primary safety objective was to assess the incidence, duration, and severity of all adverse experiences. Results: All 20 subjects and the treating investigator indicated improvement in the appearance of subjects’ lips at weeks 2, 6, and 12. SGP-HA administered for augmentation was well tolerated. Four (20%) subjects treated with SGP-HA experienced 7 treatment-emergent adverse events (TEAEs). Conclusions: Results of this study show promising efficacy and an absence of safety issues with the use of SGP-HA in lip augmentation
