Whole-tumour evaluation with MRI and radiomics features to predict the efficacy of S-1 for adjuvant chemotherapy in postoperative pancreatic cancer patients: a pilot study

Abstract: Background Multiple guidelines for pancreatic ductal adenocarcinoma (PDAC) suggest that all stages of patients need to receive postoperative adjuvant chemotherapy. S-1 is a recently emerged oral antitumour agent recommended by the guidelines. However, which population would benefit from S-1 needs to be determined, and predictors of chemotherapy response are needed for personalized precision medicine. This pilot study aimed to initially identify whether whole-tumour evaluation with MRI and radio… Show more

“…A run length refers to a sequence of consecutive pixels in a specific direction that had the same gray level. NGTDM was a method for characterizing image texture, taking into account the differences in gray level values between a pixel and the pixels in its neighborhood (Liang et al, 2021 ).…”

“…S-1 treatment has been shown to be slightly superior to gemcitabine treatment in adjuvant use for patients with resected pancreatic cancer, which helps explain its wide use for pancreatic cancer patients in many countries 11 . The efficacy of S-1 therapy is reported to be associated with the location of pancreatic cancer, features on MRI, 23 and the expression of human equilibrative nucleoside transporter 1 24 . However, the association between S-1 resistance and histopathological features remains unclear.…”

“…Proof-of-concept studies have provided promising evidence for AI in predicting responses to various treatments. Liang et al (2021) used a multivariate Cox regression model to identify radiomics features from pre-operative MRIs that could predict individual response to a new oral therapy (S-1) as an adjuvant chemotherapy and postoperative disease-free survival [95]. Tumor location and another imaging feature were significant predictors of S-1 efficacy, with mass in the pancreatic head being associated with lower treatment efficacy and survival.…”
