Whole Prostate Volume and Shape Changes with the Use of an Inflatable and Flexible Endorectal Coil

Abstract: Purpose. To determine to what extent an inflatable endorectal coil (ERC) affects whole prostate (WP) volume and shape during prostate MRI. Materials and Methods. 79 consecutive patients underwent T2W MRI at 3T first with a 6-channel surface coil and then with the combination of a 16-channel surface coil and ERC in the same imaging session. WP volume was assessed by manually contouring the prostate in each T2W axial slice. PSA density was also calculated. The maximum anterior-posterior (AP), left-right (LR), an… Show more

“…MRI articles only appeared after 2003, but MRI appeared slightly more accurate, including all three articles that directly compared TRUS and MRI. TRUS could be expected to be more operator dependant than MRI and TRUS measurements are likely to be affected more by pressure on the prostate from the balloon than by an endorectal coil (ERC), although the ERC also involves a balloon that can affect the volume [ 40 ]. MRI software may include multifeature active shape models (MFA's) which provide an accurate, automated method of planimetric measurement [ 32 ].…”

“…On the other hand, the use of endorectal balloons may affect the geometrical accuracy of treatment delivery. Studies using balloon‐type endorectal MRI coils demonstrated that endorectal balloons can cause deformations of the prostate 8,9 . Moreover, analyses of the interfraction variability in endorectal balloon placement relative to the prostate or bony landmarks reported substantial deviations in shape and position from the nominal geometry 10–12 .…”

[10][11][12] The intra-and interindividual dosimetric consequences of such residual deformations and repositioning inaccuracies on the delivered dose distributions in prostate radiotherapy with endorectal balloon are subject of this paper.…”

“…The probe and the prone position would, indeed, not alter the volume of the prostate but might deform its shape slightly. In their study, Osman et al [ 47 ] have investigated the endorectal coil effect on the WP volume and shape during prostate T2W MRI and concluded that, despite shape deformation, there is no significant change in the WP volume between including and excluding the endorectal coil. Although the needle guiding probe differs from the endorectal coil, its impact may be expected to be similar.…”
