One of the main causes of adults' disability during their working age is multiple trauma. The process of medical care of patients who are injured seriously is still a challenging job. The primary treatment of these patients in the emergency medicine departments is the most required choice after the wilderness first aid and also would be very required before definitive care in the hospital. The main aim of emergency medicine departments is quick recognition and treatment of injuries which pose severe threat to patients' life in appropriate order of priority. The procedure of primary evaluation in emergency medicine department with the help of medical routine examination and ultrasonography is based on the concept of focused assessment with sonography in trauma (FAST) for identifying spontaneous intraperitoneal hemorrhage. Emergency patients who suffer from massive hematothorax, serious lung and heart traumas, and penetrating traumas to the chest would undergo thoracotomy and patients who have few symptoms of perforated hollow viscous will undergo emergency laparotomy. Based on the trauma severity, emergency treatment could be the way to fast recovery of the structure of injured organ and its function. The subsequent goal, in the acute phase, will concentrate on preventing and stopping bleeding and secondary injuries like painful compartment syndrome or intra-abdominal infections (IAIs). However, the main aim of emergency medicine department in taking care of severely injured patients is the management of airway, protecting circulation and breathing, identification of neurologic problems, and whole body clinical examination with the help of healthcare providers.

Introduction. A computed tomography (CT) scan of the entire body has become a standard practice for the diagnosis of patients with polytrauma in many trauma centres. Purpose: evaluation of diagnostic potential of full-body CT for patients with polytrauma.Materials and methods. In the period from January 2008 to December 2018, all hemodynamically stable patients (scoring 3 and above in the Abbreviated Injury Scale (AIS) and 15 and above in the Injury Severity Score (ISS)) admitted to the Main Republican Head Injury Centre CCH №21 in Ufa with injuries in two or more anatomical areas have undergone CT scanning of head, neck (including cervical spine), chest (including thoracic spine), abdominal cavity/pelvis (including lumbar spine).Results and discussion. In the period from January 2008 to December 2018, 1498 CT examinations were conducted. Out of these examinations, 1368 and 143 cases were polytrauma — (on average 2 cases per week) and monotrauma (10 % of the study group) patients. Only 17 patients failed to complete the examination for polytrauma due to deteriorating status. All of these 17 had been returned to the intensive care unit without delay.Conclusion. In a decade 1368 patients met the established criteria for an immediate CT scan for the diagnosis of polytrauma at a large hospital providing emergency healthcare. A broad range of significant injuries was diagnosed quickly, accurately and safely. These injuries included 31 cervical spine fractures and 56 pneumothoraxes not evidenced by conventional X-ray images.

Purpose Whole-body computed tomography (WBCT) is the standard diagnostic method for evaluating polytrauma patients. When patients are unable to elevate their arms, the arms are placed along the body, which affects the image quality negatively. Aim of this systematic review is to evaluate the influence of below the shoulder arm positions on image quality of WBCT. Methods Literature in PubMed and Scopus databases was systematically searched. Results of the papers were stratified into 4 categories: arms elevated, 1 arm up 1 arm down, arms ventrally supported, arms along the body. A qualitative analysis was performed on subjective image quality and a quantitative analysis on objective quality (image noise). Results Eight studies were included with 1421 participants. Various studies reported significantly higher quality scores with arms elevated, compared to arms along the body. Significant differences in objective image quality were found between the arms elevated and the arms ventrally on support group. The arms ventrally supported group had a significantly higher image quality than the arms along the body group. A statistically significant difference was found in objective image quality between the 1 arm up 1 arm down and arms along the body group. No preferential below the shoulders position could be identified. Conclusion Positioning the arms alongside the body results in a poor image quality. Placing the arms on a pillow ventrally to the chest improves image quality. Interestingly, asymmetrical arm positioning has potential to improve the image quality for patients that are unable to elevate the arms.
