The pathophysiology of psychiatric disorders is complex and cannot be easily assessed by laboratory studies. DiŠusion tensor imaging (DTI) is a magnetic resonance imaging technique increasingly used for noninvasive and quantitative evaluation of abnormalities of the cerebral white matter because it provides exquisite details on tissue microstructures and also can perform sophisticated computer-based analyses. We review basic principles of DTI; methods of diŠusion tensor analysis; recent DTIˆndings in major psychiatric diseases, such as schizophrenia, bipolar disorder, major depressive disorder, and anxiety disorder; and factors to keep in mind when interpreting the results of DTI analysis. We recommend the frequent use of DTI as a routine clinical protocol to assess white matter abnormalities in patients with psychiatric disorders.

Bipolar disorder has been associated with anatomical as well as functional abnormalities in a brain network that mediates normal and impaired emotion regulation. Previous brain imaging studies have highlighted the subgenual cingulate (SC) and the amygdalo-hippocampal (AH) complex as core regions of this network. Thus we investigated white matter (WM) fiber tracts between the SC and the AH region, the uncinate fasciculus, as well as between two control regions (pons and cerebellum), using diffusion tensor imaging tractography in 16 euthymic bipolar patients (BP) and 16 sex-, age-and handedness-matched controls. Fractional anisotropy (FA) and apparent diffusion coefficient (ADC) of the reconstructed fiber bundle and the number of virtual reconstructed fibers were compared between groups. The tractography results revealed a significantly increased number of reconstructed fibers between the left SC and left AH in BP as compared to healthy controls. FA and ADC of the reconstructed fiber tract did not differ significantly between the groups. Furthermore, no significant group differences were observed neither for reconstructed fiber tracts between the right SC and right AH nor between the control regions. The present results suggest an altered WM pathway between the left SC and AH region and thus extend previous findings of anatomical and functional modifications in these structures in BP.

6 revista debates em psiquiatria -Mar/Abr 2015 ARTIGO DE REVISÃO NEUROIMAGEM NO TRANSTORNO BIPOLARResumo Nas últimas décadas, pesquisas de neuroimagem no transtorno bipolar (TB) têm demonstrado anormalidades nos circuitos neuronais supostamente envolvidos no processamento e na regulação da emoção, bem como no processamento de recompensas. Entretanto, os resultados relativos a diversas estruturas do sistema nervoso central são escassos e difíceis de serem comparados, devido à grande heterogeneidade do TB e às diferentes metodologias empregadas para a coleta das imagens. Esta revisão teve como objetivo sintetizar os principais achados em neuroimagem estrutural e funcional no TB, descrevendo as estruturas corticais e subcorticais do encéfalo mais relevantes e que embasam a provável fisiopatologia desse transtorno.Palavras-chave: Neuroimagem, transtorno bipolar, circuitos neuronais. AbstractIn the past few decades, neuroimaging research in bipolar disorder (BD) has demonstrated abnormalities in neural circuits thought to be involved in emotion processing and regulation, and reward processing as well. However, results related to a number of structures of the central nervous system are scarce and difficult to compare, due to both the clinical heterogeneity of BD and the different methodologies used to collect images. The objective of this review was to synthetize the main findings available in structural and functional neuroimaging in BD and to describe major cortical and subcortical brain structures that underpin the likely pathophysiology of this disorder.
