White Matter Abnormalities in Children with HIV Infection and Exposure

Abstract: Background: Due to changes in guidelines and access to treatment, more children start combination antiretroviral therapy (ART) in infancy. With few studies examining the long-term effects of perinatal HIV infection and early ART on neurodevelopment, much is still unknown about brain maturation in the presence of HIV and ART. Follow-up studies of HIV infected (HIV+) children are important for monitoring brain development in the presence of HIV infection and ART.Methods: We use diffusion tensor imaging (DTI) to … Show more

“…Two noted regionally higher fractional anisotropy (FA) and predominantly lower diffusivity in HEU compared to HUU, in both neonates and children . Higher FA was noted in the middle cerebellar peduncles and right posterior corona radiata , and lower diffusivity was noted in bilateral regions of the corticospinal tract, while the posterior corona radiata showed both significant increases and decreases in different diffusivity metrics . Abnormal Dubowitz neurobehavioral scores were positively correlated with FA in the left uncinate fasciculus and negatively correlated with diffusivity in the right inferior cerebellar peduncle and bilateral hippocampal cingulum in HEU infants .…”

“…Post‐screening, 255 full‐text articles were critically reviewed, and 25 met inclusion criteria for the systematic review (Figure ). Five articles included human subjects and studied brain size, structure and function . The remaining 20 articles were preclinical studies using animal models mostly focused on behavioural assessments (see Tables and for study characteristics and summary of outcomes).…”
