Which Test for Diagnosing Early Human Diabetic Neuropathy?

Abstract: Epidemiological studies show that diabetic neuropathy can affect 50-90% of patients with diabetes. Diabetes increases the risk of foot ulceration and amputation more than 23-fold (1), and neuropathy is the major contributory factor to this increased risk. Neuropathy presents as painful neuropathy in ;20% of patients (2), and it independently predicts all-cause and diabetes-related mortality (3).Several longitudinal studies have shown that glycemic control and vascular risk factors are important in the developm… Show more

“…Diabetic peripheral neuropathy (DPN)-the most common chronic complication of diabetes, occurs in up to 50 % of all patients with diabetes [2] and is widely considered as a main risk factor for foot ulceration and lower limb amputation [3,4]. Early detection of DPN is of great importance because it can increase opportunities for both physicians and patients to benefit from better glycemic control and to initiate strategies to prevent its complications such us foot ulcers [5,6].…”

“…Recently, determining subepithelial corneal nerve density has been promoted as a surrogate marker for early diagnosis of diabetic PN. 25 In vivo imaging of corneal nerves by corneal confocal microscopy is a noninvasive procedure, but it can be challenging to implement as a routine examination. Moreover, many images must be acquired and made into a montage for analysis, which can be very labor intensive.…”

“…Despite improvements in the management of diabetes, diabetic peripheral neuropathy (DPN) has become the most commonly reported chronic diabetic complication, affecting up to half of diabetic patients2. DPN causes serious compilations, such as foot ulcers and gangrene, leading to lower limb amputation, all of which reduce the quality of life in diabetic patients34. Several pathogenic mechanisms have been reported to be involved in DPN, including microangiopathy, oxidative stress, inflammation, the polyol pathway, glycation, and ligand activation5678.…”
