Which elements are involved in reversible and irreversible cartilage degradation in osteoarthritis?

Abstract: Osteoarthritis (OA) is a disease of the entire joint. Different treatment strategies for OA have been proposed and tested clinically without the desired efficacy. One reason for the scarcity of current chondroprotective agents may be the insufficient understanding of the patho-physiology of the joint and whether the joint damage is reversible or irreversible. In this review, we compile emerging data on cellular and pathological aspects of OA, and ask whether these data could give clue to when cartilage degrada… Show more

“…The chondrocyte is the unique resident cell of articular cartilage and thus solely responsible for ECM composition and regulation. Chondrocyte metabolism is influenced by its micro-environment, and in return influences ECM composition, organization and ultimately the mechanical resilience of cartilage [1][2][3]. As such, chondrocytes play a key role in ECM remodelling in physiological and pathological conditions [4].…”

“…With preventive strategies available to modify joint biomechanics by surgical interventions or to improve tissue resilience by pharmaceutical agents, the very earliest evidence of degeneration must be detected and adequately addressed. Early detection of degeneration is crucial as the pathology may be reversible at this point [5].…”

“…Matrix metalloproteinases (MMPs) play an essential role in the degradation of ECM proteins such as collagens and proteoglycans. 7,8 Serum MMP-3, a key enzyme in ECM remodeling, is strongly elevated in AS patients. 9,10 Increased turnover of several different MMP-derived collagen species has been accepted as a main feature of AS.…”
