What proportion of patients at the end of life contact out-of-hours primary care? A data linkage study in Oxfordshire

Abstract: ObjectivesOut-of-hours (OOH) primary care services are a key element of community care at the end of life, yet there have been no previous attempts to describe the scope of this activity. We aimed to establish the proportion of Oxfordshire patients who were seen by the OOH service within the last 30 days of life, whether they were documented in a palliative phase of care and the demographic and clinical features of these groups.DesignPopulation-based study linking a database of patient contacts with OOH primar… Show more

“…These clinical codes were validated by two members of the direct care team based on previous coding validity studies [16]. The positive predictive value (PPV) of the clinical code for medical diagnosis or conclusion was 92.6% [8]. In order to facilitate analysis these codes translated into International Classification of Primary Care (ICPC) codes by two clinically qualified authors (GH, RB) (supplementary Table 1).…”

“…In the UK, the provision of primary care services outside core contracted hours is an integral NHS service [7] , [8]. In 2013-2014, 5.8 million cases were handled by OOH GP in England resulting in 3.3 million face-to-face consultations, including 800,000 home visits [9].…”

“…Patients who had contact with the OOH service in the 30 days prior to death were categorised into those who had been documented as palliative by the service and those who had not. We defined palliative patients as those who had been assigned a clinical code specific to palliative care, been referred to a hospice or been prescribed an appropriate subcutaneous medication by the OOH service, as described fully elsewhere (3). Demographical details were compared for patients rather than contacts, so that each patient was only considered once in the analysis.…”

“…Primary care plays a significant role at the end of life, with most patients spending the majority of their last year of life in the community and preferring to die at home (1,2). Out-of-hours (OOH) GP services are involved in end of life care for approximately 30% of patients (3). Provision of palliative care outside of working hours is a research priority, however, the role of OOH services prescribing palliative medications remains poorly understood (4,5).…”
