What is the role of invasive versus non‐invasive coronary angiography in the investigation of patients suspected to have coronary heart disease?

Abstract: The improved technology of multi-slice cardiac CT angiography (CTA) has enabled production of high quality images of the coronary arteries. The sensitivity and specificity of the test in identifying patients with obstructive coronary lesions in 64-slice and later generations of scanners is high. To enable effective use of CTA in the clinical setting, a better understanding of this technology, particularly in comparison to invasive coronary angiography, is needed. In this article we discuss the characteristics … Show more

“…Chow and Sheth in their article on invasive and non‐invasive coronary angiography 1 miss in my opinion, the opportunity to offer an approach that might significantly reduce the exposure of patients with clinically diagnosed/suspected ischaemic heart disease to invasive cardiology. Given that in many situations revascularization improves symptoms but not survival, 2 non‐invasive angiography could be the only investigation of many if not most of such patients.…”

“…In contrast, coronary computed tomographic angiography (CCTA) is a non-invasive technique capable of accurately detecting obstructive CAD and evaluating atherosclerotic plaque burden in the coronary artery ( 7 , 9 - 13 ). Despite its reliability as a diagnostic tool, utilizing CCTA for coronary artery assessment remains time-consuming and requires extensive expertise ( 14 , 15 ).…”

“…Such is the dilemma we now face with how to best use a new test – computed tomography coronary angiography (CTCA). While CTCA has many potential uses, as discussed in a recent review in the Internal Medicine Journal , 3 our focus here is the use of CTCA in diagnosing coronary artery disease (CAD) in patients presenting to emergency departments (EDs) with acute chest pain who lack diagnostic features of acute coronary syndrome (ACS).…”
