Purpose: The COVID-19 pandemic resulted in the institution of strict public health measures aimed at reducing virus transmission. This coincided with a well-documented alteration to the frequency and characteristics of maxillofacial trauma presentations. The aim was to examine the incidence and characteristics of maxillofacial trauma at a large tertiary hospital in the United Kingdom to determine whether these previously reported changes persisted after the relaxation of COVID-19 “lockdown” legislation. Method: This was a retrospective cohort study of patients who sustained a facial injury during the 8-week period of 16th March 2020 to 11th May 2020 (strict COVID lockdown) compared to the 11th May 2020 to 6th July 2020 (relaxation of COVID lockdown). Cohorts were compared on the incidence and characteristics of facial injury. Results: Compared to the peak of COVID-19 lockdown, the subsequent period during which these laws were eased saw an increase in the frequency of maxillofacial trauma presentations (n = 78 vs n = 37). Despite this absolute increase, the incidence remained statistically below seasonal expectation. Operative management remained persistently low (45% in 2020 vs 67% in 2019). Conclusion: This is the first study to report the characteristics of facial trauma after the easing of COVID-19 lockdown legislation. Compared to previously published reports from Australia, there has not yet been a clear return to baseline epidemiology in the UK, perhaps reflecting the severity of the pandemic in that region.

Annotation. According to previous studies there are specific territorial, social and culturally associated patterns of prevalence and distribution of maxillofacial injuries within quarantine restrictions regarding their etiology and mechanism of occurrence in different countries, which in turn could provoke changes among individual components of generally accepted recommendations used during the treatment of patients with traumatic maxillofacial lesions under conditions of lockdown and COVID-19 pandemic. Objective – to determine the main features of the maxillofacial traumatic lesions prevalence and distribution patterns and to systematize the main changes in the protocols of specialized dental care used for the treatment of patients with traumatic lesions of the maxillofacial structures during COVID-19 pandemic. During the course of the analytical study, retrospective epidemiological data related to the prevalence of maxillofacial injuries during 2020 was processed, and comparative analysis of the obtained data with similar indicators registered during the control periods of 2018–2019 was provided. After the content analysis of available recommendations considering specifics of patients stratification, prioritization and features of certain types of surgical interventinos associated with the need for treatment maxillofacial trauma cases of varying severity, their systematization was carried out within the Microsoft Excel 2019 (Microsoft Office 2019, Microsoft) in the form of text blocks with the subsequent construction of structural and logical schemes. It was found that in the context of a global analysis provided among medical institutions that specialized at the surgical treatment of patients with various types of maxillofacial lesions, general decreasing tendency of their activity and specific reduction in elective manipulations amount were noted during 2020. The most common maxillofacial trauma cases under the conditions of quarantine restrictions were those caused by the factors of fall in house conditions and interpersonal conflicts; the amount of road traffic accident-associated maxillofacial injuries significantly decreased. There was a pronounced prioritization of those types of maxillofacial surgeries, ignoring of which could provoke life-threat for the patient; in addition, there was an increase in the number of cases of traumatic lesions treatment with closed reduction methods and a decrease in the frequency of treatment methods based on the principles of open reduction and internal fixation during COVID-19 pandemic. Thus, under the conditions of the COVID-19 pandemic a general reduction tendency regarding prevalence of maxillofacial traumatic lesions was noted, which was supported with the decrease in number of registered cases considering patients referral to the maxillofacial specialists. In order to optimize the process of providing dental care in general and to carry out appropriate surgical interventions in the maxillofacial region in particular, a number of strictly specialized, general preventive and related recommendations and directives were additionally developed.

Purpose To compare the incidence, aetiology, and patterns of maxillofacial fracture presentations during the various stages of the 2020 Melbourne COVID-19 lockdown restrictions to periods outside lockdown in 2019 and 2020. Methods This is a retrospective study of 344 subjects. The patterns of facial trauma presentations to a tertiary hospital in metropolitan Melbourne during the 2020 COVID-19 restrictions were compared to periods with no restrictions over 22 months from March 2019 to December 2020. Results The incidence of maxillofacial fractures decreased by 28% during lockdown (0.41 vs. 0.57 injuries/day, P = 0.0003). Falls overtook interpersonal violence as the leading cause of fractures (44% of lockdown presentations vs. 25.7% of presentations outside lockdown, P = 0.002), while sporting injuries dropped drastically (4% vs. 17.1%, P = 0.005). Lockdowns saw an increase in the proportion of female patients (40% vs. 26.8%, P = 0.03) and a fivefold increase in proportion of domestic violence-related fractures (6.7% vs. 1.1%, P = 0.006). Alcohol-related injuries decreased significantly (11% vs. 21%, P = 0.03). Conclusions While restrictions reduced rates of interpersonal violence and alcohol-related maxillofacial trauma, there was a higher proportion of injuries to females, increased falls, and domestic violence-related injuries.
