Objective. To assess the effectiveness of achieving and maintaining control of bronchial asthma in children during dynamic observation, depending on the characteristics of the onset of the disease. Material and methods. In order to study the peculiarities of the course and controllability of bronchial asthma in children, depending on the alternative onset of the disease, 319 children were comprehensively examined by the "experience-control" method in parallel groups using a simple random sample on the basis of the Regional Children's Clinical Hospital in Chernivtsi. In 257 patients (I clinical group), bronchial asthma developed on the background of chronic obstructive bronchitis, the II clinical group included 43 children in whom BA debuted after suffering community-acquired pneumonia, and the third (III) clinical group was formed by 19 children in whom BA was verified after inpatient treatment for status asthmaticus. Research results. It was shown that the representatives of the III clinical group, compared with other patients, probably more often had the phenotype of early asthma and severe disease, and the ratio of the chances of severe asthma in the future for these children compared with the cohort of group I was 6.8. According to the results of self-assessment of asthma control on standardized questionnaires GINA and AST, as well as using a clinical-instrumental assessment scale, it was found that the level of disease control in all groups was insufficient, and the distribution of children in clinical groups in group III relative to other patients with partially controlled and uncontrolled asthma predominated. The conducted standard anti-inflammatory basic treatment allowed to achieve certain positive changes in patients of clinical observation groups, and the representatives of the III clinical group required more active basic anti-inflammatory therapy, in particular, the volume of the 1st step in GINA compared to patients of the I group was prescribed twice less often, and the 4th - 6.9 times more often. Conclusion. Patients with the onset of bronchial asthma in the form of status asthmaticus further retain a worse level of control over the disease as compared to patients with chronic obstructive bronchitis or pneumonia in history. This cohort of patients requires a larger volume of basic treatment, as well as a personalized approach to prescribing therapy with the obligatory consideration of the characteristics of asthma onset.

The Association between Vigorous Physical Activity and Stress in Adolescents with Asthma

Asthma is a very common condition that affects 5–10% of the global population, and its prevalence is increasing. Vigorous physical activity (PA) is effective in improving cardiorespiratory fitness and managing stress. This study aimed to investigate the association between vigorous PA and stress among Korean adolescents with asthma using large-scale survey data. The questionnaire data of 57,303 adolescents were analyzed using raw data from the 2019 Korea Youth Risk Behavior Web-Based Survey. We performed logistic regression analysis to calculate the stress odds ratios (ORs) and 95% confidence intervals (CIs) for asthma and non-asthma groups using models 1 and 2. We also performed logistic regression analysis to calculate the stress OR for the asthma group with vigorous PA and non-vigorous PA using models 1, 2, and 3. Model 1 was adjusted for age, sex, obesity, smoking, and alcohol status; model 2 was further adjusted for household income, academic achievement, and comorbidities. Model 3 was further adjusted for moderate activity and resistance exercise. The OR of stress was 20% higher in the asthma group than in the non-asthma group (p < 0.05). In the fully adjusted models, the OR for the non-asthma group with vigorous PA versus non-vigorous PA was 0.89 (95% CI: 0.84–0.94). However, the OR for the asthma group with vigorous PA versus non-vigorous PA was 0.70 (95% CI: 0.57–0.86), indicating that adolescents who engage in vigorous PA had lower stress in the asthma group (p < 0.05). This study demonstrated that adolescents with asthma had higher stress levels than those without asthma; however, vigorous PA was associated with lower stress. These effects were more pronounced in adolescents with asthma.

Objectives: In recent years, the prevalence of asthma has globally increased. Despite intensive treatment, many children with asthma are not achieving good symptom control. The present study aims to analyze the prescribing trends in pediatric asthma according to the World Health Organization (WHO) prescribing indicators. Methods: This was an observational, longitudinal study conducted for the duration of 6 months with a sample size of 62. Children with age 1–14 years with asthma, who fulfilled the inclusion criteria, were enrolled after obtaining the written informed consent. Data were collected from outpatient department prescription slips for 2 times, one at 1st enrollment and the next after 1 month and changes in the prescription were noted. Peak expiratory flow rate (PEFR) was measured on both visits and the change in PEFR was noted. Results: Male predominance was seen (67.74%) with age of 5–9 years (40.32%). Average number of drugs per prescription was 1.96. The most commonly prescribed anti-asthmatic drug was inhaled corticosteroids (ICS) (78.22%). Only 1.5% drugs were prescribed by generic names and 22.22% drugs from the essential drug list. Inhalational route (38.71%) was preferred over the oral route (20.97%). After 1 month of treatment, change in PEFR was found to be statistically significant. Conclusion: The study concluded that average number of drugs per prescription and use of antibiotics was conforming to the WHO prescribing standards but prescribing from essential medicines list and by generic name need to be encouraged.
