What is Different About Serotype 1 Pneumococci?

Abstract: Serotype 1 Streptococcus pneumoniae is among the most commonly isolated serotype in invasive pneumococcal disease but is rarely found causing asymptomatic nasopharyngeal colonization. Compared to infection by other serotypes, infection caused by serotype 1 is more likely to be identified in young patients without comorbidities but is generally associated with a lower mortality. Empyema and extrapulmonary manifestations are common. Outbreaks of serotype 1 disease have been reported in closed communities and epi… Show more

“…For example, although serotype 6D has been isolated in several regions, it is characteristically prevalent in Asian countries but nearly absent in the United States (197); serotype 1 is less prevalent in southeast Asia but causes epidemics in central Africa during the end of the dry season (201). In addition, in contrast to the 15% difference in the United States and Europe (198), PPV23 coverage in Asia was estimated to be only about 5% greater than that of PCV13 owing to the relatively high prevalence of serotype 6A (200).…”

“…Serotypes with higher invasive indices can cause endemic disease, for example in the African meningitis belt [28][29][30], as well as localized outbreaks [31][32][33]. We identified only nine total pneumococci with serotypes associated with a higher invasiveness index (three at the beginning of the Hajj and six at the end).…”

“…Such strains had an early growth advantage in the blood [43] and an impaired ability to activate the NLRP3 inflammasome [44]. Interestingly, some serotype 1 strains express a non-hemolytic PLY [45;46], perhaps in part explaining its propensity for invasiveness [47]. The lack of PLY hemolytic activity appears to enable pneumococcus to escape innate immune surveillance, highlighting previous data showing the importance of PLY in promoting early bacterial clearance [41].…”
