BackgroundChange initiatives face many challenges, and only a few lead to long-term sustainability. One area in which the challenge of achieving long-term sustainability is particularly noticeable is integrated health and social care. Service integration is crucial for a wide range of patients including people with complex mental health and social care needs. However, previous research has focused on the initiation, resistance and implementation of change, while longitudinal studies remain sparse. The objective of this study was therefore to gain insight into the dynamics of sustainable changes in integrated health and social care through an analysis of local actions that were triggered by a national policy.MethodsA retrospective and qualitative case-study research design was used, and data from the model organisation’s steering-committee minutes covering 1995-2015 were gathered and analysed. The analysis generated a narrative case description, which was mirrored to the key elements of the Dynamic Sustainability Framework (DSF).ResultsThe development of inter-sectoral cooperation was characterized by a participatory approach in which a shared structure was created to support cooperation and on-going quality improvement and learning based on the needs of the service user. A key management principle was cooperation, not only on all organisational levels, but also with service users, stakeholder associations and other partner organisations. It was shown that all these parts were interrelated and collectively contributed to the creation of a structure and a culture which supported the development of a dynamic sustainable health and social care.ConclusionThis study provides valuable insights into the dynamics of organizational sustainability and understanding of key managerial actions taken to establish, develop and support integration of health and social care for people with complex mental health needs. The service user involvement and regular reviews of service users’ needs were essential in order to tailor services to the needs. Another major finding was the importance of continuously adapting the content of the change to suit its context. Hence, continuous refinement of the change content was found to be more important than designing the change at the pre-implementation stage.

Se presentan los resultados generales de una investigación sobre trabajo interprofesional en la implementación de programas sociales, discutiéndolos a la luz de los impactos de la racionalidad neoliberal y desde una perspectiva que pone las discusiones sobre justicia social como un horizonte político insoslayable en el momento actual. Basado en la aplicación de un diseño mixto de investigación, incluyendo técnicas de producción de datos cuantitativas y cualitativas, los resultados del estudio sugieren que el trabajo interprofesional presenta una serie de nudos críticos que se relacionan con esta racionalidad neoliberal: las condiciones laborales inestables, sobrecargadas y precarias de las/os profesionales, la confianza excesiva en su capacidad emprendedora, la construcción de un estatus desigual entre las profesiones y la reproducción de lógicas individualistas y competitivas en las prácticas cotidianas.

This article documents the presence of policy transfer on integrated care development, its global occurrence and shifts towards integrated care. It highlights the influence of supranational forces, the roles of policy transfer, and the relevance of policy translation in the development of integrated care. Design/methodology: This article presents the findings of an international review of the policy transfer of integrated care, and the relevance of policy translation in integrated care development. Findings: The global occurrence in integrated care, as evinced in this article, can be seen in the global shift towards integrated care in various countries. However, studies exploring the actual mechanism of policy transfer and policy translation in relation to integrated care across countries are limited. The study of integrated care through the lens of policy transfer is important, as it for example, explores the structural elements, including environmental and cognitive obstacles in the policy transfer process. Policy translation offers a social constructivist approach to explore the travel of ideas, and considers the multiple spatial and scalar contexts in which integrated care policy is implemented. Originality/value: This article aims to advance policy transfer and policy translation as complementary frameworks to explain integrated care development. Second, it seeks to make novel and useful contributions to the debate about the development of integrated care, and to the wider arguments on policy transfer and policy translation and integrated care in other parts of the world.
