Introduction: Negative attitude towards drug therapy can foster limited adherence to treatment, which remains one of the biggest obstacles for implementing effective treatments, especially long term. Purposes: The purposes of the study were 1) to evaluate the attitude towards drug therapy among a representative sample of patients treated in a community psychiatric service using 30-item Drug Attitude Inventory (DAI-30); 2) to evaluate the DAI-30 dimensions, applying factorial analysis; and 3) to highlight the socio-demographic and clinical variables correlated to DAI-30 score and factors. Methods: The DAI was administered, over a 7-month period, to all patients treated in our psychiatric outpatient services who agreed to participate in this study and provided their informed consent. Data were statistically analyzed. Results: With a response rate of 63.3%, 164 females and 136 males completed the DAI-30 with an average score of 14.24 (±10.46 SD), indicating moderately positive attitude towards drug therapy. The analysis of DAI-30 internal consistency confirmed its reliability (Cronbach's alpha=0.84). Our factorial analysis highlighted three factors: Factor 1 (Cronbach's alpha=0.81), composed of 7 items which indicate positive, trustful attitude; Factor 2 (Cronbach's alpha=0.78), composed of 5 items indicating negative attitude of suspiciousness; and Factor 3 (Cronbach's alpha=0.66), composed of 4 items suggesting defensive and control attitude towards drug therapy. Discussion: Among the selected variables, "monotherapy" and "total number of hospitalizations" were negatively correlated to the final score of DAI-30, whereas being "married" was positively correlated to it, in a statistically significant way, using the multiple linear regression model. These correlations suggest that positive attitude towards drug therapy could be reinforced by the condition of being married and reduced by relapses with hospitalization, as literature highlighted, and, paradoxically, by a monotherapy, which could suggest a sort of psychological dependence on therapy and, indirectly, on psychiatric service, potentially correlated to the long-term treatments of our patients.

Background: The recovery model in mental health care emphasizes users’ right to be involved in key decisions of their care, including choice of one’s primary mental health professional (PMHP). Aims: The aim of this article was to provide a scoping review of the literature on the topic of users’ choice, request of change and preferences for the PMHP in community mental health services. Method: A search of the PubMed, Cochrane Library, Web of Science and PsycINFO for papers in English was performed. Additional relevant research articles were identified through the authors’ personal bibliography. Results: A total of 2,774 articles were screened and 38 papers were finally included. Four main aspects emerged: (1) the importance, for users, to be involved in the choice of their PMHP; (2) the importance, for users, of the continuity of care in the relationship with their PMHP; (3) factors of the user/PMHP dyad influencing users’ preferences; and (4) the effect of choice on the treatment outcomes. Conclusion: While it is generally agreed that it is important to consider users’ preferences in choosing or requesting to change their PMHP, little research on this topic is available. PMHPs’ and other stakeholders’ views should also be explored in order to discuss ethical and practical issues.

Objective: The aim of the study was to compare users of community-based mental health day centres (DCs) and clubhouses in Sweden regarding reported social networks and social interaction and the stability of these over time. A further aim was to investigate social network predictors both cross-sectionally and longitudinally.Methods: People regularly attending DCs (n=128) or clubhouses (n=57) completed questionnaires about social network and social interaction (social engagement and social functioning), self-esteem and socio-demographics at baseline and a nine-month follow-up.Results: Perceived social engagement and social functioning did not differ between the groups and remained stable over time. Fewer in the DC reported having a close friend but there was no difference regarding having recently (the past week) seen a friend. When naming "someone with whom you can share your innermost thoughts and feelings", the DC group named more professional contacts, fewer friends and more often "nobody" compared to the clubhouse group. Finally, on both occasions the DC group scored significantly lower on size of the social network compared to the clubhouse users. Self-esteem and having recently seen a friend could predict size of the social network in the cross-sectional perspective. Strong indicators of belonging to the group with a larger social network at follow-up were being a woman, attending a clubhouse programme and having scored high on social network at baseline. Conclusion and Implication for Practice:Having friends and strengthening one's self-esteem may be essential factors for the social network of people with psychiatric disabilities in a short-term perspective. Visiting clubhouses seems advantageous in a longer-term perspective.
