Introduction: In patients with hypoparathyroidism, conventional therapy maintains parathyroid hormone (PTH)-dependent mineral metabolism homeostasis but is unable to prevent emergence of comorbidities and low quality of life. Objectives: To evaluate long-term progression of patients with hypoparathyroidism receiving conventional therapy and their quality of life compared with patients with primary hypothyroidism and healthy controls. Design and Setting: Retrospective cohort study for quality-of-life analysis and transversal cut on clinical profile. Patients with hypoparathyroidism from four public referral centers in endocrinology and bone metabolism in the metropolitan region of Rio de Janeiro. Material and Methods: Quality of life by SF-36 protocol, and clinical profile by medical record analysis. Results: 243 individuals with hypoparathyroidism (n=113), hypothyroidism (n=65), and healthy controls (n=65) included. Median time since diagnosis and duration of conventional therapy was 8 years (IQR 4–17 years). Data on type of conventional therapy (median, minimum–maximum daily dose, percentage of patients with hypoparathyroidism using each medication): calcium supplementation (2000 mg/day, 200–6000 mg/day, 95%), cholecalciferol (2000 IU/day, 200–40000 IU/day, 44%), calcitriol (0.5 μg/day, 0.25–2 μg/day, 77%), thiazides (25 mg/day, 12.5–100 mg/day, 44%). Conclusions: Conventional therapy is associated with homeostasis of serum mineral levels, but not with improved quality of life. Compared to patients with hypothyroidism, those with additional hypoparathyroidism had lower scores in six SF-36 domains. Conventional therapy successfully maintained normal calcium levels with often high doses of calcium, vitamin D, and thiazides but could not prevent low quality of life scores and comorbidities.

Well differentiated thyroid cancer is a common malignancy diagnosed in young patients. The prognosis tends to be excellent, so years of survivorship is expected with low risk disease. When making treatment decisions, physicians should consider long-term quality of life outcomes when guiding patients. The implications for treating indolent, slow growing tumors are immense and warrant careful consideration for the functioning years ahead. Surgery is the standard of care for most patients, however for a subset of patients, active surveillance is appropriate. For those wishing to treat their cancer in a more active way, novel remote access approaches have emerged to avoid a cervical incision. In the era of “doing less”, options have further expanded to include minimally invasive approaches, such as radiofrequency ablation that avoids an incision, time off work, a general anesthetic, and the possibility of post-treatment hypothyroidism. In this narrative review, we examine the health related quality of life effects that surgery has on patients with thyroid cancer, including some of the newer innovations that have been developed to address patient concerns. We also review the impact that less aggressive treatment has on patient care and overall wellbeing in terms of active surveillance, reduced doses of radioactive iodine (RAI) treatment, or minimally invasive techniques such as radiofrequency ablation (RFA) for low risk thyroid disease.

Impairments in quality of life and predictors of symptom burden in patients with hypoparathyroidism: results from a population-based survey

Purpose To investigate the quality of life (QoL) in patients with hypoparathyroidism (hypoPT) compared to the general population and to identify sociodemographic and clinical factors that are associated with symptom burden. Methods Patients with a diagnosis of hypoPT participated in an online survey. Information regarding the survey was distributed by treating physicians or a self-help organization. Quality of life was assessed using the EORTC QLQ-C30 and symptom burden using the Hypoparathyroid Patient Questionnaire (HPQ28). Multivariate linear regression analysis was used to compare QoL of hypoPT patients with the general population (adjusted for age, sex, education)and to identify factors associated with symptom burden. Results Altogether, 264 hypoPT patients provided information on QoL and symptom burden. HypoPT was associated with worse cognitive (β = −15.9; p < 0.01) and emotional functioning (β = −12.3; p = 0.04) compared to the general population. The highest symptom burden in hypoPT patients was observed for the domains loss of vitality (mean: 61.4; SD: 21.9), pain and cramps (mean: 43.7; SD: 26.5), and numbness and tingling sensations (mean: 38.9; SD: 30.0). Female gender was associated with a higher symptom burden across all nine domains of the HPQ28, while longer disease duration was associated with a lower symptom burden in neurovegetative symptoms, loss of vitality, depression and anxiety, and depressive symptoms. Conclusion HypoPT patients have impaired QoL compared to the general population. Being female is strongly associated with high symptom burden.
