Background Informal caregivers commonly experience mental health difficulties related to their caregiving role. e–Mental health interventions provide mental health support in a format that may be more accessible to informal caregivers. However, e–mental health interventions are seldom implemented in real-world practice. Objective This mixed methods systematic review aimed to examine factors associated with the effectiveness and implementation of e–mental health interventions for informal caregivers of adults with chronic diseases. To achieve this aim, two approaches were adopted: combinations of implementation and intervention characteristics sufficient for intervention effectiveness were explored using qualitative comparative analysis, and barriers to and facilitators of implementation of e–mental health interventions for informal caregivers were explored using thematic synthesis. Methods We identified relevant studies published from January 1, 2007, to July 6, 2022, by systematically searching 6 electronic databases and various secondary search strategies. Included studies reported on the effectiveness or implementation of e–mental health interventions for informal caregivers of adults with cancer, chronic obstructive pulmonary disease, dementia, diabetes, heart disease, or stroke. Randomized controlled trials reporting on caregivers’ mental health outcomes were included in a crisp-set qualitative comparative analysis. We assessed randomized controlled trials for bias using the Risk of Bias 2.0 tool, and we assessed how pragmatic or explanatory their trial design was using the Pragmatic Explanatory Continuum Indicator Summary 2 tool. Studies of any design reporting on implementation were included in a thematic synthesis using the Consolidated Framework for Implementation Research to identify barriers to and facilitators of implementation. Results Overall, 53 reports, representing 29 interventions, were included in the review. Most interventions (27/29, 93%) focused on informal cancer or dementia caregivers. In total, 14 reports were included in the qualitative comparative analysis, exploring conditions including the presence of peer or professional support and key persuasive design features. Low consistency and coverage prevented the determination of condition sets sufficient for intervention effectiveness. Overall, 44 reports were included in the thematic synthesis, and 152 barriers and facilitators were identified, with the majority related to the intervention and individual characteristic domains of the Consolidated Framework for Implementation Research. Implementation barriers and facilitators in the inner setting (eg, organizational culture) and outer setting (eg, external policies and resources) domains were largely unexplored. Conclusions e–Mental health interventions for informal caregivers tend to be well-designed, with several barriers to and facilitators of implementation identified related to the intervention and individual user characteristics. Future work should focus on exploring the views of stakeholders involved in implementation to determine barriers to and facilitators of implementing e–mental health interventions for informal caregivers, focusing on inner and outer setting barriers and facilitators. PurposeThis secondary data analysis explores the unmet needs of adolescents experiencing maternal cancer in Ireland. Research has shown that one of the challenges adolescents deal with at the time of maternal cancer is having unmet needs that can impact negatively on their experience and their ability to cope through this difficult challenge.Design/methodology/approachFifteen adolescents completed qualitative interviews as part of a larger study that explored the experience of adolescent adjustment to maternal cancer. The transcripts of these original interviews were analysed using a secondary content analysis underpinned by the categories included in the Offspring Cancer Needs Instrument (Patterson et al., 2013).FindingsThe findings of this study suggest a necessity to individually explore the unmet needs of adolescents as these were not uniform even within a small sample of 15 adolescents. Unmet needs change and evolve over time as does maternal illness. Adolescents themselves identified the need for more education in the general public and in clinical practitioners on how to respond appropriately to their needs. It is crucial that adolescent's needs and emotions are validated at the time as part of the support provided for them.Originality/valueThis study provides important recommendations for practice and policy on how to provide tailored supports for adolescent who experience cancer in their families as currently there is a lack of effective and evidence-based targeted supports for this specific age group.

Background: Parental cancer affects not only the patient, but all family members. Access to psychological support for the entire family is highly relevant, but still scarce. The aim of the present research project was to develop, implement, and evaluate the effectiveness of a short-term counselling intervention for families with parental cancer. Methods: The short-term counselling intervention consisted of six sessions and was developed based on pre-existing evaluated concepts. All participating family members had to complete questionnaires prior and post-intervention. These questionnaires investigated changes in family communication and satisfaction levels, parental anxiety and depression levels, parental partnership quality and self-assessed quality of life of children and adolescents. Furthermore, feasibility of and satisfaction with the intervention was assessed. Results: Acceptability of short-term counselling was high in participating families, although feasibility was limited with regard to the final inclusion rate (n = 10). Comparison of pre and post intervention measures showed a significant increase in family communication and satisfaction levels. Anxiety and depression levels were low, quality of parental partnership within the normal range, and quality of life reported from children was high. These scores remained stable from pre to post intervention. Conclusions: The counselling intervention most likely had positive effects in all family members with either stabilizing or increasing all measured outcomes. Although interpretation of the results is limited due to the small sample size, we believe that integrated in clinical routine of adult care of a hospital, this additional offer might be a valuable benefice to cancer patients and their families.
